Intervenir sur les presses à imprimer : validation d’un arbre de défaillance associé aux risques mécaniques

Click here for the English version of this report_x000D_ En 2006, un recensement des pratiques d'usage les plus sécuritaires, relatives à des interventions sur des presses à imprimer, a été effectué par l'Association paritaire de santé et de sécurité du travail secteur imprimerie et activités connexes (ASP imprimerie), dans le but d'établir une procédure de travail sécuritaire propre à ces machines qui comportent plusieurs zones dangereuses. À partir de cela, l'ASP a construit un arbre de défaillance retraçant les combinaisons de causes engendrant l'écrasement d'une partie du corps d'un travailleur dans un angle rentrant de presse à imprimer. Par conséquent, cette étude se concentre sur les risques d'écrasement liés aux angles rentrants de ces machines._x000D_ Grâce à cet arbre de défaillance, l'ASP imprimerie a conçu une méthode d'évaluation de risques liés à des opérations sur ces presses. Mise à l'épreuve lors de visites auprès d'environ 25 entreprises du secteur, l'application de cette méthodologie s'est avérée concluante. Propulsée par ce résultat positif, l'ASP imprimeriea adressé une demande d'expertise à l'équipe de recherche de l'IRSST afin de valider la logique et l'exhaustivité de l'arbre pour que l'ASP puisse le diffuser par la suite, avec une méthodologie d'évaluation des risques et de procédures sécuritaires quant à l'exécution des quatre opérations suivantes :_x000D_ _x000D_ Le nettoyage et le lavage des rouleaux et des blanchets,_x000D_ L'insertion et le retrait des plaques,_x000D_ L'insertion et le retrait des blanchets,_x000D_ L'enfilage du papier._x000D_ _x000D_ La présente étude visait à valider la logique et l'exhaustivité de l'arbre de défaillance initial par l'équipe de l'IRSST. Cette validation a été réalisée selon la méthodologie suivante :_x000D_ _x000D_ Recherche documentaire pour s'informer davantage sur la notion d'arbre de défaillance, sur les presses à imprimer, sur les quatre opérations à l'étude ainsi que sur les normes et la réglementation en vigueur applicables aux presses à imprimer;_x000D_ Validation sur le terrain : visites de huit presses à imprimer pour mieux comprendre le danger relatif aux angles rentrants auquel fait face l'opérateur lors des quatre opérations;_x000D_ Achèvement et correction de l'arbre de défaillance de l'ASP, vérification de sa structure pour y apporter des corrections;_x000D_ Validation de l'arbre de défaillance corrigé avec l'ASP imprimerie._x000D_ _x000D_ De l'application de cette méthodologie, découle le résultat de l'étude soit un arbre de défaillance final avec 300 causes combinées, occupant 10 niveaux et pouvant expliquer l'écrasement d'une partie du corps d'un travailleur par un ou des rouleaux/cylindres de presse à imprimer lors d'une opération. Cet arbre est un outil utile. D'une part, il permet de visualiser les causes et conséquemment de trouver des solutions pour éviter un tel écrasement lors d'une opération. L'efficacité des solutions dépendra de la catégorie d'appartenance des causes sur lesquelles on décidera d'agir. D'autre part, bien que cet arbre de défaillance ne soit pas directement transposable à d'autres machines ayant des angles rentrants et pour lesquelles on voudrait étudier les causes d'un écrasement dans un angle rentrant, il reste un très bon modèle de départ et pourrait être adapté, moyennant un minimum d'effort._x000D_ AbstractIn 2006, a survey of the safest practices relating to interventions on printing presses was carried out by the Association paritaire de santé et de sécurité du travail secteur imprimerie et activités connexes (ASP imprimerie, joint sector-based occupational health and safety association – printing and related activities sector) to establish a safe work procedure for these machines with several danger zones. From this, the ASP developed a fault tree tracing the combinations of causes resulting in a part of a worker's body being crushed in a printing press nip point. Consequently, this study focuses on the crushing hazards related to nip points on these machines._x000D_ Because of this fault tree, the ASP imprimerie developed a method for evaluating the hazards related to operations on these presses. Tested during visits to approximately 25 companies in the sector, the application of this methodology proved conclusive. Encouraged by this positive result, the ASP imprimeriemade a request for expertise to the IRSST's research team to validate the fault tree's logic and comprehensiveness so that the ASP could then disseminate it, with a risk evaluation methodology and safe procedures for performing the four following operations:_x000D_ _x000D_ Roller and blanket cleaning and washing,_x000D_ Plate insertion and removal,_x000D_ Blanket insertion and removal,_x000D_ Paper threading._x000D_ _x000D_ The aim of this study was for the IRSST team to validate the initial fault tree's logic and comprehensiveness. This validation was conducted according to the following methodology:_x000D_ _x000D_ Literature search to obtain more information about the concept of fault tree on printing presses, on the four operations studied, as well as on the standards and regulations in force applicable to printing presses;_x000D_ Field validation: visits to eight printing presses to better understand the nip point hazards that the operator has to deal with during the four operations;_x000D_ Completion and correction of the ASP fault tree, and verification of its structure so that corrections could be made to it;_x000D_ Validation of the corrected fault tree with ASP imprimerie._x000D_ _x000D_ Application of this methodology generated the result of the study, namely a final fault tree with 300 combined causes, occupying 10 levels and that can explain the crushing of a part of a worker's body by one or more rollers/drums of the printing press during an operation. This tree is a useful tool because the causes can be visualized, and consequently, solutions can be found to avoid crushing accidents during an operation. The effectiveness of the solutions will depend on which category of causes is chosen for action. However, while this fault tree is not directly transposable to other machines with nip points and where the causes of crushing in a nip point need to be studied, it remains a very good starting model and could be adapted with minimum effort.
