Le diabète de type 2 est caractérisé par une combinaison d'hyperinsulinémie/résistance à l'insuline et une diminution de la sécrétion d'insuline par les cellules betas pancréatiques. Outre l'hyperglycémie, principale responsable des complications microvasculaires, le diabète de type 2 s'accompagne de dyslipidémie et d'hypertension artérielle (HTA) qui augmentent les risques de complications macrovasculaires, première cause de mortalité chez ces patients.Le traitement doit donc optimiser trois paramètres: l'équilibre glycémique, le bilan lipidique et la tension artérielle. Les objectifs de l'étude ont donc été (1) d'évaluer les effets d'un programme de traitement multifactoriel intensif sur l'équilibre glycémique (glycémie à jeun et hémoglobine glycosylée (HbA[indice inférieur 1c])), la tension artérielle et le profil lipidique (cholestérol total, LDL cholestérol, HDL cholestérol, triglycérides et ratio cholestérol total/HDL) chez des patients diabétiques de type 2 mal contrôlés en visant les objectifs de traitement de l'Association canadienne du diabète (ACD), (2) d'évaluer les effets du traitement multifactoriel intensif sur la qualité de vie des patients et (3) de vérifier si les effets du traitement multifactoriel intensif persistent à court terme (six mois après l'arrêt du programme). Les patients randomisés au groupe intensif (n = 36) ont bénéficié pendant 12 mois du traitement multifactoriel intensif comprenant des visites mensuelles avec examen clinique et biologique, éducation, ajustements thérapeutiques touchant la diète, l'exercice physique et les médications, un programme d'exercice physique d'intensité progressive, une diète contrôlée et deux appels téléphoniques par mois pour recevoir les résultats de la visite, ajuster les thérapeutiques si nécessaire et maintenir la motivation. À 12 mois, le traitement multifactoriel intensif a démontré des effets bénéfiques significatifs sur l'équilibre glycémique, la tension artérielle systolique, le cholestérol total, les triglycérides, le ratio cholestérol total/HDL ainsi que sur la qualité de vie. Six mois après l'arrêt du programme, les bénéfices ont été maintenus dans le groupe intensif pour la glycémie à jeun, la tension artérielle diastolique et le profil lipidique, mais l'HbA[indice inférieur 1c], la tension artérielle systolique et le poids ont augmenté. Il n'y a plus de différence entre les groupes d'étude sauf pour le cholestérol total et le LDL cholestérol. L'étude montre donc que le traitement multifactoriel intensif est efficace et qu'il permet de contrôler les facteurs de risques cardiovasculaires des patients diabétiques les plus à risque. Il faudrait sans doute que ces programmes soient maintenus pour que les acquis persistent. L'étude montre aussi que le traitement multifactoriel intensif est acceptable par les patients, car malgré son intensité, la qualité de vie des sujets s'améliore.--Résumé abrégé par UMI.
