Cliquez ici pour la version française de ce rapport_x000D_ Les problèmes de santé mentale au travail représentent actuellement l’une des plus importantes causes d’absence au travail, et ce phénomène a connu une croissance marquée au cours des deux dernières décennies. Les travaux entrepris dans le domaine de la réadaptation professionnelle et de la santé mentale au travail ont permis de mettre en évidence l’importance d’une démarche de collaboration entre les acteurs concernés par le processus de retour au travail. Toutefois, si quelques études ont porté précisément sur la réintégration professionnelle de travailleurs déjà insérés en emploi et qui se sont absentés en raison d’un problème de santé mentale, très peu d’entre elles ont porté sur la conception et l’implantation de programmes de retour au travail._x000D_ Ce document porte sur la conception, l’implantation et l’évaluation d’un programme intégré de pratiques de soutien au retour au travail à la suite d’une absence en raison d’un problème de santé mentale. Le projet de recherche découle d’une demande paritaire faite par un établissement de santé et de services sociaux du Québec pour intervenir sur la problématique des absences et du retour au travail, liée à la santé mentale, au sein de son personnel. Pour ce projet, l’équipe de recherche a mis en place une démarche paritaire à plusieurs volets, en concertation avec un comité-conseil, représentatif de l’ensemble des acteurs concernés. Le mandat du comité-conseil était de participer à une définition consensuelle du problème et des moyens à mettre en œuvre pour soutenir le retour au travail et le maintien en emploi des personnes à la suite d’une absence pour des raisons de santé mentale. Ce comité avait également le mandat d’assurer l’élaboration d’une démarche qui s’appuie sur la cohérence des pratiques entre les différents acteurs du milieu et de convenir des stratégies aptes à favoriser la mobilisation de l’ensemble du personnel de l’organisation autour de cette problématique. Son travail a mené à la conceptualisation d’une théorie de programme et à sa modélisation opérationnelle._x000D_ La méthode utilisée pour mettre au point le programme de retour au travail à la suite d’une absence pour des raisons de santé mentale s’appuie sur une démarche structurée de collectes d’informations à partir d’une analyse des données d’absence et d’entrevues auprès des acteurs engagés dans le processus de retour au travail afin de déterminer leurs pratiques actuelles; la consultation de documents organisationnels en matière de retour au travail; une recension de la littérature scientifique sur le sujet; une prise en compte des pratiques courantes mises en place dans les milieux de travail concernés par le problème. Ces renseignements ont permis de conceptualiser la théorie de programme composée d’un modèle théorique, d’un modèle logique et d’un modèle opérationnel. Avec la collaboration du comité-conseil, l’équipe de recherche a mis au point une démarche d’accompagnement et de soutien au rétablissement et au retour au travail destinée aux employés absents de leur travail pour des raisons de santé mentale. Cette démarche visait à cibler les facteurs cliniques, personnels et organisationnels qui ont contribué à leur arrêt de travail. La recherche de solutions pour aider les employés à revenir et à se maintenir en emploi a été pour la plupart d’entre eux, un exercice positif. De plus, l’importance accordée à l’accueil, au soutien au travail, à la mise en œuvre de pratiques de rassurance pendant l’arrêt de travail et à la possibilité pour les personnes d’apporter des changements dans le travail apparaît ici être des déterminants pour un retour au travail réussi._x000D_ Cette recherche a également permis de mettre en évidence que la réussite d’un tel programme de retour au travail ne peut être assurée sans que des conditions essentielles soient présentes: l’établissement d’un climat de confiance marqué par le respect, une communication efficace et une collaboration entre les différents acteurs internes ou externes visés par le retour au travail. Finalement, il est aussi intéressant de constater que les interventions effectuées auprès des personnes absentes peuvent également toucher les autres travailleurs exposés à ces mêmes facteurs de risque. Ultimement, l’approche individuelle d’une telle démarche pourra passer à une démarche organisationnelle touchant les autres travailleurs exposés. Ce rapport de recherche présente donc en détail les différentes étapes de la recherche et les principaux résultats et constats qui en ont découlé._x000D_ Abstract_x000D_ Mental health problems in the workplace are currently one of the main causes of work absence, a phenomenon that has seen a marked increase in the last two decades. Research work in the occupational rehabilitation and mental health field has highlighted the importance of a collaborative process involving the stakeholders in the return-to-work process. However, while a handful of studies have focused on the work reintegration of workers who held jobs but had been absent from them for mental health reasons, very few of these studies have specifically explored the design and implementation of return-to-work programs._x000D_ This report concerns the design, implementation, and evaluation of an integrated practices program for supporting a return to work following an absence for a mental health problem. The research project was carried out in response to a joint employee/employer request made by a health and social services establishment in Québec, Canada, for intervention regarding the problem of mental health-related work absences among its personnel and their return to work. To carry out the project, the research team utilized a multi-faceted employee/employer approach and worked with a research steering committee representing all the stakeholders. The steering committee’s terms of reference were to assist in arriving at a consensus definition of the problem and of the means to be implemented to support both the return to work of workers who had been absent for mental health problems and their job retention. The committee was also responsible for developing a process based on consistent practices among the various workplace stakeholders and for reaching agreement on strategies conducive to the active involvement of all the organization’s personnel in this issue. Its work led to the conceptualization of a program theory and the development of an operational model._x000D_ The method used to develop the program for supporting a return to work following an absence for mental health reasons had several components: a structured information-collection process that involved the analysis of absence data and of interviews with stakeholders engaged in the return-to-work process in order to determine their current practices; consultation of organizational documents on the return-to-work process; a review of the scientific literature on the topic; and an examination of current practices in workplaces concerned by this problem. This information laid the groundwork for conceptualizing the program theory in the form of a theoretical model, a logic model, and an operational model. With the steering committee’s collaboration, the research team developed a process for assisting in and supporting the recovery and return to work of employees who were absent from work for mental health reasons. This process was aimed at identifying the clinical, personal, and organizational factors that had contributed to their stopping work. For most of the employees, the search for solutions to help them return to work and retain their jobs was a positive experience. The importance placed on welcoming back the returning employees and on supporting them in their work, implementing reassurance practices during their sick leave, and allowing them to make changes in their work would also appear to be a determining factor in a successful return to work._x000D_ This study further revealed that the success of such a program can be ensured only if the following essential conditions are in place: a climate of trust characterized by respect, effective communication, and collaboration among the various internal and external stakeholders involved in the return to work. Lastly, it should be noted that interventions carried out with absent workers may also have an impact on other workers exposed to the same risk factors. Ultimately, the individual approach used in such a process could become an organizational process involving the other exposed workers. This research report details the various steps in the study and the main results and findings.
