OBJETIVO: Determinar el valor de las Reglas de Tobillo de Ottawa para el diagnóstico de fractura en traumatismos agudos de tobillo en la Clínica Good Hope – Lima en el periodo julio – octubre 2016. \nMATERIAL Y METODOS: Se realizó un estudio observacional, prospectivo, analítico, transversal de pruebas diagnósticas que evaluó 267 pacientes con lesiones agudas de tobillo entre junio a octubre del 2016. \nRESULTADOS: El promedio de edad de la muestra estudiada fue de 35.08, con un 55.1% de personas del sexo masculino y 44.9% del sexo femenino. De los 267 pacientes fueron 75 los que presentaron fractura de tobillo (28.1%). Dentro de los mecanismos de lesión más frecuentes se observó que las caídas representaron el 43.1% de casos, seguido por la torsión en 39.7% y accidentes de tránsito en 17.2%. De los pacientes que presentaron fractura de tobillo el mecanismo de lesión más frecuente fue las caídas (45.3%) continuando con los accidentes de tránsito (29.3%) finalizando con la torsión (25.3%). Un 9.7% de todos los pacientes estudiados se encontraba realizando actividades deportivas. De los que presentaron fractura de tobillo, el 34.7% se encontraba realizando algún tipo de actividad deportiva. Las reglas del tobillo de Ottawa fueron positivas para el 63.3% de los pacientes estudiados con un 36.7% de negatividad. Las reglas del tobillo de Ottawa fueron positivas en el 100% de los pacientes que presentaron fractura de tobillo. El rendimiento de las reglas de Ottawa para predecir fractura de tobillo tuvo una sensibilidad del 100%, especificidad de 51%, valor predictivo positivo de 44.4% y valor predictivo negativo de 100%. \nCONCLUSIONES: Las Reglas del tobillo de Ottawa mostraron alta confiabilidad para decidir cuándo tomar radiografías en las lesiones de tobillo y adecuado rendimiento para diagnosticar fracturas de tobillo. Los resultados de este estudio no muestran falsos negativos y están de acuerdo con los resultados de otros estudios similares. Nos anima a aplicar estos criterios en nuestros servicios con emergencia.
