Abstract
Akuuttien aivoverenkiertohäiriöiden diagnostiikassa käytetään rutiininomaisesti varjo-ainetehostamatonta pään tietokonetomografiatutkimusta (TT). Monileike-TT-tekniikan kehittäminen on mahdollistanut aivo- ja kaulavaltimoiden varjoainekuvauksen eli TT-angiografian ja veren läpivirtausta luonnehtivien TT-perfuusiotutkimusten käyttämisen liuotushoitoarvion tukena.\n\nTässä tutkimuksessa sovellettiin Alberta Stroke Program Early CT Score (ASPECTS) -pisteytysjärjestelmää 92:n akuuttiin aivoverenkierron häiriöön sairastuneen potilaan perfuusiokarttojen tulkinnassa. Perfuusion tunnuslukujen käyttökelpoisuutta selvitettiin akuutin aivoverenkiertohäiriön ja liuotushoidon tehokkuuden arvioinnissa.\n\nTutkimus osoitti, että aivokudoksen alentuneen veritilavuuden (CBV) pinta-ala arvioi hyvin lopullisen aivoinfarktin kokoa ja että pinta-ala korreloi CBV-ASPECTS-pisteiden kanssa. Palautuvasti iskeemisen kudosalueen (penumbra eli puolivarjo) laajuuden arvi-oimisessa ASPECTS-pisteiden erotus CBV:n ja veren pidentyneen keskimääräisen kauttakulkuajan (MTT) välillä osoittautui käyttökelpoiseksi.
