L’outil d’identification de la situation de handicap au travail (OISHT) consiste en une entrevue structurée qui a été développée pour aider les cliniciens à identifier de façon systématique les facteurs liés à l’incapacité au travail. L’OISHT est un des rares outils disponibles en réadaptation pour la clientèle en phase chronique d’incapacité au travail. Il tient compte de l’influence de différents systèmes (personnel, santé, travail, indemnisation) et a été élaboré pour les deux principales problématiques de santé associées à l’incapacité au travail, soit les troubles musculosquelettiques (TMS) et les troubles mentaux courants (TMC). L’objectif de cette étude visait la validation de l’OISHT auprès de personnes absentes du travail, en raison de TMS ou de TMC. Plus précisément, il s’agissait de décrire les qualités psychométriques suivantes de l’OISHT : 1) la validité de construit, 2) la cohérence interne, 3) la fidélité interjuges et 4) la validité convergente. Un devis corrélationnel transversal a été utilisé avec un échantillonnage non probabiliste. Les critères d’inclusion des ergothérapeutes pour participer étaient : 1) être membres de l’Ordre des ergothérapeutes du Québec; 2) avoir au moins une année d’expérience en réadaptation au travail; 3) avoir suivi la formation spécifique pour utiliser l’OISHT (durée moyenne de 4 heures). Quant aux travailleurs ayant participé, ils devaient : 1) être absent du travail en raison d’un TMS ou TMC depuis au moins trois mois, mais moins de deux ans; 2) avoir un lien d’emploi et 3) être admis dans un programme de réadaptation. L’administration de l’OISHT durait entre 90 et 120 minutes. De plus, pour l’évaluation de la validité convergente, les participants ont rempli des questionnaires autoadministrés mesurant différents concepts associés. Aussi, pour un sous-groupe de participants de chaque population, des ergothérapeutes ont complété, entre 24 et 72 heures après l’utilisation de l’OISHT, un Inventaire des causes du maintien de l’incapacité au travail (n = 72 pour la version TMS et n = 65 pour TMC). Finalement, six ergothérapeutes ont coté l’OISHT à partir d’histoires de cas standardisés afin d’évaluer la fidélité interjuges pour chaque population à l’étude. Au total, 35 ergothérapeutes ont participé à l’évaluation de 290 participants, soit 140 personnes avec TMS et 150 avec TMC. Pour la validité de construit, une analyse factorielle exploratoire (AFE) a permis de dégager des dimensions semblables au sein des deux versions, c’est-à-dire la représentation de la maladie, le jugement clinique en regard de la sévérité de la condition médicale et le niveau d’exigences élevées du travail. Cette analyse a permis de réduire d’environ 20 à 40 % le nombre d’items, selon les versions. Cependant, les résultats variables pour la cohérence interne de certaines dimensions, issues de l’AFE, suggèrent certaines réserves, principalement pour la version pour la population présentant des TMC. Quant à la fidélité interjuges, elle est satisfaisante en regard de la nature de l’outil, qui est une entrevue structurée. Enfin, en l’absence de mesure étalon, la validité convergente reste à confirmer. En somme, malgré les limites rapportées, il s’avère que le contenu de l’OISHT, en réduisant le nombre d’items proposés, correspond bien aux écrits scientifiques actuels sur les populations à l’étude et s’inscrit dans une compréhension biopsychosociale de la personne en incapacité de travail. Toutefois, la version pour la population de personnes présentant des TMC demande à être révisée et testée à nouveau. Abstract The Work Disability Diagnosis Interview (WoDDI) consists of a structured interview designed to help clinicians systematically identify the factors contributing to a work disability. It is one of few tools available in rehabilitation for use with individuals in the chronic phase of work disability. It takes into account the influence of various systems (personal, health care, workplace, and compensation), and was developed for the two main health problems associated with work disability: musculoskeletal disorders (MSDs) and common mental disorders (CMDs). The aim of this study was to validate the WoDDI with individuals absent from work due to an MSD or a CMD. More specifically, it sought to describe the following psychometric properties of the instrument: (1) construct validity, (2) internal consistency, (3) interrater reliability, and (4) convergent validity. A cross-sectional correlational design was used with a non-probability sample. The inclusion criteria for occupational therapists were (1) membership in the Ordre des ergothérapeutes du Québec, (2) at least one year’s experience in work rehabilitation, and (3) prior specific training in the use of the WoDDI (average duration of four hours). The inclusion criteria for workers were (1) being absent from work for at least three months but less than two years due to an MSD or a CMD, (2) having a contractual relationship with their employer, and (3) being enrolled in a rehabilitation program. It took between 90 and 120 minutes to administer the WoDDI. In addition, for the purpose of evaluating the instrument’s convergent validity, the participants completed self-report questionnaires measuring various related concepts. For a sub-group of participating workers, the occupational therapists also completed (between 24 and 72 hours after administering the WoDDI) an inventory of causes of work disability maintenance (n=72 for the MSD version and n=65 for the CMD version). Lastly, to assess interrater reliability for the two populations under study, six occupational therapists used the WoDDI to score standardized case histories. In total, 35 occupational therapists took part in the evaluation of 290 participating workers, including 140 with an MSD and 150 with a CMD. Regarding the exploratory factor analysis (EFA) of the instrument’s construct validity, the results revealed similar dimensions in both versions, specifically, illness representation, clinical judgment of the seriousness of the medical condition, and high levels of work demands. This analysis made it possible to reduce the number of items in the instrument by approximately 20 to 40%, depending on the version. However, the varying results obtained from the EFA for the internal consistency of certain dimensions suggest certain limitations, mainly for the version designed for the CMD population. Interrater reliability was satisfactory as regards the nature of the instrument (i.e., a structured interview). Lastly, given the lack of a standard measure, the convergent validity still has to be confirmed. In summary, despite the WoDDI’s reported limitations, by reducing the number of items included, its content appears to correlate well with the current scientific literature on the two populations under study. It also fits in with a biopsychosocial understanding of individuals on work disability. However, the CMD version of the instrument requires revision and retesting.
