Introduction : la chirurgie des dissections aortiques de type A selon la classification de Stanford est associée à un haut risque de morbidité et de mortalité. Les facteurs prédictifs, les facteurs pronostiques et les coûts d'une prise en charge prolongée de ces patients en secteur de soins intensifs de chirurgie cardiaque n'ont pas encore été étudiés. Méthode : cette analyse a inclus 3538 patients de la base de données ERTAAD ( European registry of Type A aortic Dissection), comprenant tous les patients ayant bénéficié d'une chirurgie pour dis.section aortique aigue de type A puis d'une prise en charge en soins intensifs de chirurgie cardiaque, entre janvier 2005 et mars 2021, dans 17 centres de chirurgie cardiaque répartis dans huit pays européens. Le critère de jugement principal était la mortalité intra-hospitalière. Résultats : la durée moyenne de séjour en soins intensifs de chirurgie cardiaque était de 9.9 ±9.5 jours. Le coût total moyen d'une prise en charge en soins intensifs de chirurgie cardiaque était de 24 086 ±32 084 €. La mortalité intra-hospitalière dans cette série était de 14,8% et la mortalité à cinq ans était de 30.5%. Conclusion : un séjour prolongé en soins intensifs de chirurgie cardiaque est courant en postopératoire de chirurgie pour dissection aortique de type A. Dans notre étude, il s'agit d'un facteur associé à une meilleure survie jusqu'à cinq ans après le geste chirurgical.
