Geriatrik bireylerde farklı ikili görev eğitimlerinin denge, düşme korkusu ve yaşam kalitesine etkisi

Abstract
Amaç : Geriatrik bireylerde entegre ve ardışık ikili görev eğitimlerinin denge, düşme korkusu, yürüme hızı, ikili görev maliyeti, bilişsel durum ve yaşam kalitesine etkilerinin incelenmesidir.Gereç ve yöntem : 58 katılımcı rastgele Entegre İkili Görev (EİG) ve Ardışık İkili Görev (AİG) grubu olarak ayrılmıştır. 8 hafta, haftada 2 gün uygulanan eğitimlerin öncesi ve sonrasında; Berg Denge Ölçeği (BDÖ), Zamanlı Kalk-Yürü Testi Standart (ZKY-ST), Zamanlı Kalk-Yürü Testi Bilişsel (ZKY-BİL), Tinneti Düşme Etkinlik Ölçeği (TDÖ), 10 metre Yürüme Hızı Tekli Görev (10m YH-TG), 10 metre Yürüme Hızı Bilişsel (10m YH-BİL), İkili Görev Maliyeti Hesaplaması (İGM), Montreal Bilişsel Değerlendirme Ölçeği (MOCA), Yaşlılar için Dünya Sağlık Örgütü Yaşam Kalitesi Anketi (Whoqol-Old) kullanılmıştır. Bulgular : Tekli görev denge, yürüme hızı ve yaşam kalitesi parametrelerinde eğitim modellerinin benzer düzeyde etkin olduğu saptanmıştır (p<0,05). Grup-zaman etkileşiminde; BDÖ, ZKY-ST, 10mYH-TG, 10mYH-BİL, İGM, Whoqol-Old ölçeklerinde istatistiksel olarak fark saptanmamışken, ZKY-BİL, TDÖ, MOCA ölçeklerinde fark olduğu ve değişim oranının EİG grubunda daha fazla olduğu saptanmıştır (p<0.05). Grup içi değişime bakıldığında; MOCA skorunun yanlızca EİG grubunda anlamlı düzeyde değiştiği, 10m YH-TG ölçeğinin her iki grupta gelişmediği görülmüştür (p>0.05). Grupların diğer skorları istatistiksel olarak anlamlı düzeyde değişmiştir (p<0.05).Sonuç : Sonuçlarımız, özellikle aktiviteleri eş zamanlı yapmakta zorlanan geriatrik bireylerde ardışık ikili görev eğitiminin de denge yeteneği, yürüme hızı ve yaşam kalitesinin artırılmasında, düşme korkusunun azaltılmasında kullanılabilecek güvenli bir alternatif olduğunu göstermektedir.
