Abstract
Low back pain (LBP) in the general population is as a leading cause of life-quality reducing and shows important socioeconomic consequences .Various researches are attempted to identify predictive factors to diminish the impact of LBP and prevent the passage into chronicity (cLBP).Association between psychosocial factors, physical outcomes and age represents well-studied risks factors for LBP's progression into chronicity.However, while recent studies reported high level of LBP prevalence in elderly population (65+ years), the relation between psychosocial factors and physical LBP outcomes has not been investigated extensively in that age group.The objectives of the present master project were to evaluate the impact of aging on the relation between specific psychosocial factors and physical outcomes and to compare data between elderly with and without cLBP.It intends to expose a relationship between psychosocial predictors and isometric trunk endurance and exertion results in order to provide clinical standardized tools and data for efficient early detection of cLBP among elderly patients and efficient classification of cLBP severity.Sixt y-one community-dwelling elderly individuals, aged 61 to 89 years, volunteered to participate in this study; twenty-nine with cLBP.Experimentations were conducted on a one time session in a laboratory setting and lasted about two hours.Participants first completed measures of fear of movement, catastrophizing, depression, pain seve rit y and perceived disability.They were then asked to perform maximal isometric trunk exertions (MITEx) and three maximal isometric trunk endurances (MITEn) (a modified Sorensen test) on a 3D ' Roman chair, in prone as weil as left and right side positions.Dependant variables included LBP's duration and origin, limited daily activities' duration v caused by LBP, pain seve rit y, disability, depression, fear of movement, catastrophizing and pain anticipation at a modified Sorensen test.We found that elderly with cLBP showed reduced endurance time and trunk muscle strength compared to healthy elderly.Elderly with cLBP also demonstrated higher levels of fear of movement, pain catastrophizing, depression, disability and pain intensity.In elderly with cLBP, significant relation between level of leisure and endurance times, pain catastrophizing and maximal exertion in side-bridge as weil as association between level of disability and maximal exertion in prone position were observed.In participants without cLBP, lower endurance times were associated with a high level of fear avoidance.This study highlights the importance of psychological factor in the evolution of low back pain for in elderly population.Evaluation and treatment of cLBP in older population should involve psychosocial factor and physical outcome evaluations.vi REMERCIEMENTS Merci toutes les personnes qui ont contribu au bon droulement de mon projet de matrise.Je tiens remercier mon directeur de recherche, le Professeur Martin Descarreaux, de m'avoir pris sous son aile et accord tout ce temps pour l'avancement de mes travaux.Je suis particulirement reconnaissante pour la patience et la confiance qu 'il m'a accordes.Sa passion contagieuse pour la recherche , ses commentaires pertinents et son encadrement constant m'auront encourag poursuivre mes recherches et continuer dans la bonne direction .Dans ma carrire future, je me souviendrai de ses bons conseils au sujet d'un travail efficace et d'un raisonnement scientifique et tenterai de m'en approcher le plus possible.Merci de m'avoir permis de raliser ce projet en me procurant et m'indiquant o trouver les ressources ncessaires.Merci toute l'quipe de recherche en adaptation/radaptation neuromusculaire (GRAN) pour votre prcieux soutien la ralisation de ce projet, mes collgues de laboratoire Jean-Daniel Dubois, Jean-Philippe Pia lasse et Annick Champagne pour vos prcieux conseils et assistance, mes amies et collgues la matrise Julie Q'Shaughnessy et Sana Driss pour vos encouragements et votre prsence inestimable tout au long de mes tudes de cycles suprieurs.Je ne remercierai jamais assez ma famille pour ses encouragements et sa comprhension qui m'auront, une fois de plus, permis de passer travers une tape exigeante mais importante de mon cheminement acadmique et professionnel.Merci
