Les conséquences dramatiques d'une forte exposition prénatale à l'alcool sont bien connues, mais les effets d'une faible consommation d'alcool pendant la grossesse sont sujets à controverse et les recommandations officielles à ce sujet diffèrent entre les pays. En France, 23% des femmes continuent à boire de l'alcool durant leur grossesse. Nous avons réalisé une revue critique de la littérature sur l'association entre une faible consommation d'alcool durant la grossesse et les effets sur le fœtus. Une recherche a été menée de Septembre 2012 à Juillet 2013 dans les principales bases de données. Les articles correspondant aux critères d inclusion prédéfinis, ont été récupérés et évalués en terme de qualité avec l'échelle Newcastle-Ottawa. Parmi les 4655 titres résultant de la recherche, 11 publications correspondaient aux critères d'inclusion. Il n'a pas été démontré d'association entre une consommation maternelle d'alcool inférieure à 2 verres standards par semaine et un risque accru de mort fœtale, de troubles de la croissance, de prématurité ou de malformation fœtale. Il existait néanmoins d'importantes faiblesses méthodologiques et une grande hétérogénéité parmi les études de cette revue. Dans l'état actuel des connaissances, le choix le plus sûr pour la femme enceinte est de ne pas consommer d'alcool. Ce message doit être donné par tous les professionnels de santé avec une information des conséquences de l'alcool sur l'enfant. Des études à forte puissance avec un protocole standardisé et une évaluation précise du modèle de consommation devraient étudier les conséquences d'une faible exposition prénatale à court, moyen et long terme.
