Rinoviruksen merkitys astman kehittymisessä lapsilla

Abstract
Syventävien opintojen kirjallisessa työssäni olen tarkastellut rinoviruksen (RV) aiheuttaman varhaisen uloshengitysvaikeuden yhteyttä myöhemmin lapsuudessa puhkeavaan astmaan. Olen laatinut kyseisestä aiheesta kirjallisuuskatsauksen perehdyttyäni rinovirukseen liittyvään kirjallisuuteen ja viimeisimpään tutkimusnäyttöön rinoviruksen merkityksestä lasten astman kehittymisessä. \n \nPienet lapset sairastuvat virusten aiheuttamiin hengitystieinfektioihin useita kertoja vuodessa. Rinoviruksen on jo kauan tiedetty olevan merkittävin ylähengitystieinfektioiden aiheuttaja ja myöhemmin sen tärkeä rooli on myös osoitettu alahengitystieinfektioissa. Rinovirukset jaetaan kolmeen lajiin: RV-A, RV-B ja diagnostiikkamenetelmien kehittymisen myötä hieman myöhemmin tunnistettu RV-C. RV-serotyyppejä on yli 160 erilaista. \n \nEri tutkimusten mukaan 30–50 % lapsista kärsii uloshengitysvaikeudesta ainakin kerran ennen kouluikää. Varhaislapsuudessa sairastettu rinovirusinfektion laukaisema uloshengitysvaikeus on huomattava riskitekijä toistuviin uloshengitysvaikeuskohtauksiin. Uloshengitysvaikeus luokitellaan erilaisiin fenotyyppeihin sen mukaan, missä iässä uloshengitysvaikeutta esiintyy. Oleellista on, että kaikilla kolmella tyypillä on havaittu yhteys myöhemmin lapsuudessa puhkeavan astman riskiin. \n \nTarkkaa mekanismia, jolla rinovirus vaikuttaa astman kehittymiseen, ei ole vielä saatu selville. Tutkimuksissa on osoitettu rinovirusinfektioiden liittyvän mm. heikentyneeseen interferonivasteeseen, korostuneeseen kasvutekijöiden tuotantoon ja atooppiseen tulehdukseen, joiden seurauksena hengitysteiden vaurioituminen ja uudelleen muotoutuminen tehostuvat. Tutkimustyötä tarvitaan vielä lisää, jotta mekanismit astman kehittymisen taustalla tarkentuvat. \n \nAstma on yksi lasten yleisimmistä pitkäaikaissairauksista, minkä vuoksi tutkimus rinovirusinfektioiden ennaltaehkäisy- ja hoitokeinojen kehittämiseksi on ensiarvoisen tärkeää. Rokotetta ei ole vielä onnistuttu kehittämään, sillä RV-serotyyppien suuri määrä luo haasteen rokotekehitykselle. Rinovirusinfektion laukaisemaa uloshengitysvaikeutta on tärkeää pyrkiä hoitamaan alusta alkaen tehokkaasti, jotta toistuvat uloshengitysvaikeuskohtaukset ja mahdollinen astman kehittyminen saataisiin estettyä.
