Caractéristiques de la douleur et variables associées à la santé mentale dans une population de patients souffrant de douleur chronique suivis en consultation externe de psychiatrie

Abstract
L'association entre la douleur chronique (DC) et la santé mentale est bien établie dans la littérature. Bien que quelques études se soient penchées sur la prévalence de la DC chez la clientèle psychiatrique, peu de données sont disponibles au sujet des facteurs qui influencent la sévérité des symptômes dépressifs et anxieux chez cette clientèle. Dans cette optique, la présente étude visait à: 1) Décrire les caractéristiques cliniques de la douleur, l'utilisation de moyens pharmacologiques et non pharmacologiques pour remédier aux symptômes douloureux, les stratégies d'ajustement face à la douleur, le fonctionnement psychologique, la qualité de vie et la qualité du sommeil des patients souffrant de DC suivis en consultation externe de psychiatrie, 2) Comparer les caractéristiques de la douleur et l'état de santé des patients souffrant et ne souffrant pas d'un trouble de l'humeur ou d'un trouble anxieux, et 3) Identifier les variables associées à la sévérité des symptômes dépressifs et anxieux chez cette même clientèle. Pour répondre à ces objectifs, une étude transversale a été réalisée dans une population de patients souffrant de DC suivis en consultation externe de psychiatrie au Cabinet Médical Spécialisé États d'Esprit de Rouyn-Noranda, Québec, entre février 2009 et août 2010. Les sujets étaient admissibles pour l'étude s'ils étaient âgés d'au moins de 18 ans et s'ils souffraient de symptômes douloureux depuis au moins 3 mois. Les données ont été recueillies grâce à un questionnaire autoadministré ainsi qu'une évaluation réalisée par le clinicien. Les principales variables à l'étude étaient mesurées grâce à des échelles standardisées et validées. Un total de 126 patients remplissaient les critères de sélection parmi lesquels 92 ont participé à l'étude (taux de participation = 73%). Les résultats de notre étude suggèrent que l'état de santé physique est loin d'être optimal, ce qui souligne l'importance de sensibiliser les professionnels de la santé à la prévalence, aux caractéristiques et aux conditions associées à la DC chez cette clientèle. Il est souhaitable que les professionnels de la santé œuvrant dans le milieu de la psychiatrie ciblent certains de ces aspects pour améliorer la prise en charge de la santé mentale de ce type de clientèle. \nThe association between chronic pain (CP) and mental health is well established in the literature. Altough few studies have evaluated the prevalance of CP in psychiatric populations, little is known about factors that influence the severity of depressive and anxious symptoms in these patients. Therefore, the objectives of the present study were to: 1) Describe the clinical characteristics of pain, utlization of pharmacological and non-pharmacological pain treatments, pain copin strategies, psychological functioning quality of life, and quality of sleep of CP patients followed in an outpatient psychiatry setting, 2) Compare the characteristics and the health status of patients who are suffering and who are not suffering from depressive of anxious symptoms, and 3) Identify the variables associated with depressive and anxious symptoms severity in these patients. To meet these objectives, a cross-sectional study on CP patients followed at États d'Esprit outpatient psychiatry clinic, Rouyn-Noranda, Québec, was conducted between February 2009 and August 2010. Patients were eligible if they were at least 18 years of age and reported pain symptoms for at least 3 months. The data were collected with a self-administered questionnaire and a clinician evaluation. The main study outcomes were mesured with standardized and validated scales. A total of 126 patients were eligible and were approached to participate in the study. Of those, 92 participated in the study (participation rate = 73%). Our results suggest that the health status of CP patients followed in an outpatient psychiatry setting is less than optimal, highlighting the importance of healthcare professionals should consider some of these factors to improve the management of these patients' mental health.
