INTRODUCCIÓN: La fractura periprotésica de cadera es una patología relativamente infrecuente, de difícil manejo y potencialmente grave, sobre todo si tenemos en cuenta la morbimortalidad y la disfuncionalidad que acarrea. Está en relación con mecanismos de baja energía y hay multitud de factores que aumentan el riesgo de padecer una fractura periprotésica. OBJETIVO: El objetivo principal de este estudio es conocer el perfil epidemiológico y las características de las fracturas periprotésicas en nuestra población (sector III de la provincia de Zaragoza). Otros objetivos son comparar la mortalidad de nuestro hospital respecto a otras series publicadas y conocer y cuantificar los factores de riesgo que se asocian con la mortalidad de los pacientes intervenidos de fractura periprotésica en nuestro Servicio. MATERIAL Y MÉTODOS: Se ha realizado un estudio retrospectivo observacional descriptivo y analítico de una muestra tomada del HCU Lozano Blesa desde octubre 2010 a octubre 2016. La muestra es de 75 pacientes. RESULTADOS: De los 75 participantes 26 son hombres (34.7%) y 49 mujeres (65.3%). La edad media fue 81.01 (78.89-83.14), los pacientes estuvieron ingresados de media 15.23 días (desviación típica 7.93). La media de tiempo en meses desde la implantación de la prótesis hasta la fractura fue de 99.78 (79.1-109.47). De los 75 casos a estudio fallecieron durante el seguimiento 33 pacientes (44%). La media de tiempo medida en meses desde que sucede la fractura periprotésica hasta el fallecimiento de los pacientes fue de 18.21 (11.35-25.05). DISCUSIÓN Y CONCLUSIONES: El estudio de las fracturas periprotésicas es complejo debido a los múltiples factores involucrados en su etiopatogenia. Las fracturas periprotésicas son más frecuentes en mujeres, en pacientes con factores de riesgo médicos como la toma de algunas sustancias y factores quirúrgicos relativos al implante. La clasificación de Vancouver define la fractura y determina el tratamiento, aunque actualmente está en entre dicho y es el cirujano el que decide finalmente optar por una osteosíntesis o un recambio en las fracturas de tipo B sin estar del todo claro qué es lo más adecuado. Es una patología que presenta gran mortalidad por lo que se requiere una actuación sobre los factores de riesgo para evitarla.
