Abstract
Amaç: Çalışmanın amacı ataksi semptomları olan bir olguda sanal gerçeklik uygulamasının etkisini araştırmaktır.Gereç ve yöntem: Araç içi trafik kazası sonrası kafa travması teşhisi ile takip edilen olgunun değerlendirmesinde spinoserebellar ataksiyi işaret eden denge kaybı, dizartri, tremor problemleri bulunmuştur.2 yıl süreyle tedaviye alınan hastanın progresyonunda plato evreye ulaşıldığı için sanal gerçeklik uygulaması hasta tedavisi için planlanmıştır.Değerlendirmelerde ataksi ile ilgili olarak dengesel olan ve olmayan koordinasyon testleri, fonksiyonel uzanma testi, performans analizi için Kanada Aktivite Performans Ölçümü (Canadian Occupational Performance Measure -COPM) kullanıldı.Sanal gerçeklik uygulaması içinde yer alan skorlar kaydedildi.Olgunun rehabilitasyon programında amaca uygun içeriğe sahip iki oyun seçilerek haftada bir seans, 30 dakika boyunca sanal gerçeklik tedavisi uygulandı.Sonuçlar: COPM'de olgu tarafından belirtilen günlük yaşam aktivitelerindeki 7 aylık çocuğunu kucağına alıp yürümek, alış verişe gitmek, traş olmak istediğini belirtti ve performans puanlarını sırasıyla 5, 5 ve 4 olarak; memnuniyet puanlarını ise 1 olarak bildirdi.Uygulama sonunda statik çift bacak duruş süresi 30 sn'den 2.15 dk'ya; fonksiyonel uzanma testi sonuçları sol el için 18 cm'den 22 ve sağ el için 19'den 23'ye arttığı görülmüştür.Ancak tek bacak duruş süresinde 2 saniyelik gerileme, COPM sonuçlarında da değişme olmadığı kaydedilmiştir.Tartışma: Olgumuza uygulanan sanal gerçeklik uygulamasının ataksi ile ilgili semptomların azalmasında etkili olduğu düşünülmektedir.Denge, koordinasyon gibi vestibular sistem becerilerini artırmak için uygun sanal gerçeklik uygulamaları ile klinik olarak bireylere gelişme sağlanabilir.Ancak kullanılan oyunların günlük hayatta kullanılan aktiviteleri içermesi ile bireylerin rehabilitasyon sürecinde günlük hayata adaptasyonunu kolaylaştıracağı düşünülmektedir.
