Il volume affronta la storia dell'AIDS in Africa, molto diversa da quella avutasi in Occidente dove dal 1996 erano disponibili le cure. One World, One Hope: il motto con cui alla Conferenza di Vancouver furono annunciate finalmente efficaci cure contro l'HIV non ebbe seguito pratico. Le cure si diffusero in buona misura in Africa solo verso il 2010, al costo di milioni di morti evitabili se gli standard sanitari fossero stati più uniformi e i pregiudizi sulle società africane non avessero frenato dal trattarne i malati. Malgrado l'afro-pessimismo, l'opzione terapeutica si sarebbe infine imposta, di contro a una scienza medica e a una cooperazione internazionale inizialmente scettiche e refrattarie.
