Certaines situations complexes de patients âgés admis en réadaptation posent des problèmes d'élaboration du projet de sortie et d'orientation dans le réseau de soin. Un "colloque de réseau" réunissant le patient, ses proches, et les différents professionnels institutionnels et communautaires est proposé pour ces situations. Son but est de faciliter l'élaboration du projet d'orientation et d’améliorer la coordination de la prise en charge ultérieure. Dans ces colloques de réseau, les moyens sont donnés à chacun de participer et la décision finale résulte de la concertation entre tous les participants. La perception que les personnes âgées et leurs proches ont de cette réunion n'est pas connue.
