Introduction : la recherche liée au trouble cognitif léger (TCL) fournit des informations sur les mécanismes de la maladie au stade pré-démence et permet le développement d’examens complémentaires ayant pour but une détection précoce (PET-amyloïde, biomarqueurs du LCR). Cela a pour but d'établir des stratégies de prévention et de développer des interventions thérapeutiques et non thérapeutiques pour le TCL et la démence. Cependant, il n’existe pas à ce jour de test de dépistage suffisamment sensible et spécifique. Cette étude a pour but de déterminer si le MCE est supérieur au MMSE et au MoCA pour discriminer les patients des témoins. Matériel et méthode : la discrimination du MCE concernant le dépistage des TCL a été comparée au MMSE et au MoCA avec comme examen de référence diagnostic la batterie GRECogVASC, par analyse de régression logistique ajustée sur les facteurs de confusion (âge, niveau socio-culturel). Résultats : le MoCA est sélectionné pour discriminer les patients des témoins (n=91 ; OR : 0.618, IC95 % : 0.556-0.688 ; p=0.000) ; pour discriminer les patients AVC (n=45) des témoins (n=508) et les patients Dégénératifs (n=46) des témoins (n=508). On retrouve un effet plafond du MMSE et du MoCA alors que le MCE présente une distribution d’allure gaussienne. Discussion et conclusion : ces résultats ont montré que le MoCA est plus discriminant que le MCE et le MMSE. Cependant, les patients inclus dans la pré-étude du MCE étaient constitués d’AVC uniquement. Le calibrage du MCE semble trop stricte diminuant la capacité de discrimination du test. Il faudrait donc revoir les techniques de calibration d’autant plus que certains subtests semblent plus discriminants que d’autres.
