Stili difensivi, alessitimia e percezione della qualita della vita in pazienti affette da neoplasia al seno

Abstract
Introduzione: Il presente lavoro indaga la qualità delle reazioni emotive e psicologiche delle pazienti affette da cancro mammario, sottoposte a intervento chirurgico e in diversi stadi di malattia. Metodologia: Il campione è composto da 110 pazienti così suddivise: 40 in follow-up, 41 sottoposte a chemioterapia adiuvante, 29 metastatiche. Strumenti: Defense Mechanisms Inventory (DMI, Ihilevich e Gleser, 1986), Toronto Alexithymia Scale (TAS-20, Taylor GJ,1992), EORTC QLQ-C30-BR23 (Fayers PM et al 1998). Risultati: Nei tre gruppi si rivela differenza alle scale F1 (p<.01), F2 (p<.03), F3 (p<.00) della TAS-20 e QoL (p<.02) e FS (p<.01) dell’EORTC. Al DMI si riscontrano punteggi più elevati alla scala PRN; nelle pazienti anziane, con livello di scolarità inferiore e disoccupate si osserva una riduzione del punteggio alla scala TAO e un aumento alle scale TAS e REV. Le scale TAO e TAS sono correlate negativamente (p<.00), c’è, invece, una correlazione positiva tra il punteggio delle scale TAS e F2, REV e F3 (p<.00; p=.03). Conclusioni: Le pazienti metastatiche presentano un più alto tasso di alessitimia e peggiore percezione della qualità della vita. La differenza nel profilo difensivo e nella gestione delle emozioni non dipende dallo stadio della malattia quanto da variabili quali età, scolarità e occupazione. Le pazienti utilizzano uno stile difensivo che permette di sganciare gli aspetti emotivi da quelli cognitivi padroneggiando questi ultimi attraverso un controllo intellettuale, ma quelle anziane, con bassa scolarità e disoccupate, rivolgono l’aggressività contro se stesse e negano gli aspetti emotivi. Questo stile difensivo si associa con la difficoltà ad esprimere le emozioni.
