Características clínicas y paraclínicas de la meningitis bacteriana en niños. Estudio en cinco centros hospitalarios de referencia en la ciudad de Bogotá

Abstract
INTRODUCCIÓN: como existen muchas dificultades para llegar al diagnóstico de la meningitis bacteriana en niños, se decidió evaluar la conducta que se seguía en diferentes centros hospitalarios de la ciudad. OBJETIVO: describir las características clínicas y paraclínicas de los casos de meningitis bacteriana, atendidos en cinco centros hospitalarios de referencia de la ciudad de Bogotá, entre el 1 de enero y el 31 de diciembre del 2003. MÉTODOS: estudio descriptivo retrospectivo de pacientes con diagnóstico de egreso de meningitis bacteriana y edades entre un mes y 15 años, en cinco centrosde referencia de Bogotá RESULTADOS: se incluyeron 55 niños de los hospitales la Misericordia, San Rafael, Kennedy, San Ignacio y Clínica Colsubsidio. Los síntomas fueron: fiebre 92 por ciento, vómito 78 por ciento, irritabilidad 52 por ciento, cefalea 40 por ciento y convulsiones 30 por ciento. La mitad presentó signos meníngeos. Recibieron antibióticos previos al ingreso el 26 por ciento. La aglutinación en látex se realizó en 65 por ciento, fue negativa en el 70 por ciento de casos. La tinción de Gram fue negativa en el 52 por ciento de los pacientes. El 55 por ciento de los cultivos fueron negativos y la bacteria más aislada fue el S. pneumoniae. Los hemocultivos fueron negativos en 65 por ciento de los casos. Sólo un paciente falleció. Los antibióticos más utilizados fueron la ceftriaxona y la vancomicina. CONCLUSIONES: los casos confirmados de meningitis alcanzaron el 50 por ciento. La clínica es útil, es frecuente la presencia de signos meníngeos. Deben revisarse los criterios clínicos y paraclínicos utilizados en el medio para asegurar un diagnóstico preciso.
