Le fratture periprotesiche dell'anca sono una complicanza sempre più frequente nelle persone anziane sottoposte a sostituzione protesica, con implicazioni cliniche significative per la gestione chirurgica e il recupero post-operatorio. Questo studio osservazionale, condotto tra il 2018 e il 2024, ha analizzato un campione di 94 pazienti con frattura periprotesica dell'anca. Lo studio ha integrato dati retrospettivi e prospettici per valutare i risultati clinici, le complicanze post-operatorie e i fattori che influenzano il recupero funzionale. I pazienti sono stati selezionati in base a criteri specifici, inclusa la classificazione della frattura secondo il sistema Vancouver. Le variabili studiate comprendevano età, tipo di impianto, durata dell'intervento, durata del ricovero, tempo passato tra frattura ed intervento, tecnica chirurgica, deambulazione pre e post-operatoria, complicanze entro sei mesi, trasfusioni di sangue eseguite ed altro. I risultati hanno evidenziato una significativa correlazione tra la classificazione Vancouver e la tipologia di trattamento chirurgico. Le fratture di tipo B2 e B3 richiedevano frequentemente revisioni protesiche, mentre le fratture di tipo B1 erano gestite principalmente con la riduzione e la sintesi. L'analisi statistica ha mostrato che la durata dell’intervento e del ricovero erano maggiori nei pazienti con fratture complesse. Nonostante il 65% dei pazienti abbia recuperato il livello di deambulazione pre-frattura, il 34% ha riportato un declino funzionale, mentre il tasso complessivo di complicanze, inclusa la mortalità entro sei mesi, è stato del 21,3%. La correlazione negativa tra l'età e la durata dell'intervento supporta l'ipotesi che nei pazienti più anziani si tenda a ridurre la complessità degli interventi per minimizzare i rischi chirurgici. Questo studio sottolinea l'importanza di una gestione personalizzata delle fratture periprotesiche, che consideri non solo la gravità della frattura ma anche lo stato generale del paziente, al fine di migliorare gli esiti clinici e ridurre le complicanze post-operatorie.
