Clostridium difficile fue descrito inicialmente como un componente de la flora intestinal normal de los neonatos. Sin embargo, no fue hasta 1978 cuando se le identificó como una importante causa de colitis pseudomembranosa asociada al uso de clindamicina. Este agente es responsable de un espectro de enfermedades denominado infección por C. difficile (ICD), que va desde un cuadro de diarrea no complicada y colitis pseudomembranosa hasta el megacolon tóxico y, en ocasiones, sepsis e incluso muerte. Aunque la ICD está asociada sobre todo al uso de los antibióticos, su epidemiología ha cambiado en las tres últimas décadas. Durante los años 2002 a 2005 se identificó en la provincia de Quebec (Canadá) una epidemia de ICD causada por la cepa BI/NAP1/027. Desde entonces en numerosos países se han comunicado brotes regionales de ICD, de colitis grave e incluso de muerte, causados por esta cepa. Además, estas infecciones se están empezando a identificar en poblaciones previamente no consideradas en riesgo, tales como sujetos sin contacto reciente con el sistema de atención sanitaria, embarazadas, niños, y pacientes con enfermedad inflamatoria intestinal.
