Strategies for preventing prolonged disability in workers compensated for work related musculoskeletal disorders: A systematic and comprehensive literature review

Abstract
Les troubles musculo-squelettiques d'origine professionnelle (TMSOP) représentent un lourd fardeau économique pour notre société en raison du nombre sans cesse croissant de journées de travail perdues par un faible pourcentage de travailleurs dont l’incapacité au travail se prolonge. La prolifération continue des concepts dans le domaine de l'incapacité au travail, la diversité des interventions et des contextes de mise en œuvre, de même que les intérêts variés des intervenants impliqués dans le processus de réadaptation et de retour au travail (RAT) des travailleurs indemnisés pour TMSOP posent des défis de taille aux chercheurs et aux praticiens actifs dans le domaine de l'incapacité au travail. Les buts premiers de cette revue de la littérature étaient de fournir un aperçu des principaux thèmes et tendances dans la recherche contemporaine en ce qui concerne l’intervention en prévention de l'incapacité au travail, et d'identifier les modalités d'intervention susceptibles de contribuer à la prévention de l'incapacité prolongée au travail chez les travailleurs indemnisés pour TMSOP. Une recherche électronique systématique des articles publiés en anglais ou en français entre janvier 2000 et janvier 2008 a été réalisée dans dix bases de données scientifiques. Les descripteurs et les mots clés recherchés dans les bases de données se rapportaient à cinq concepts clés: avec l'absentéisme au travail, les TMSOP, les interventions en matière d'incapacité au travail, les interventions en matière de réadaptation et de RAT, et les questions d'indemnisation. Des analyses descriptives ont été effectuées sur 186 articles traitant d'une ou plusieurs études révélatrices, sur 20 articles théoriques et sur 12 revues de la littérature. Afin d'identifier les principaux thèmes et les tendances dans les études, les articles ont été classés en fonction du principal objectif de l'étude, du ou des intervenants impliqués dans l'étude ou intéressés par les résultats de l'étude, du domaine d'application et de la perspective de recherche. Des analyses de contenu narratives ont ensuite été réalisées à partir des 88 articles identifiés par les auteurs comme contenant des descriptions des éléments stratégiques d'intervention visant à prévenir une incapacité prolongée chez les travailleurs indemnisés pour troubles musculo-squelettiques. Les principaux thèmes abordés dans les études sur l'incapacité au travail concernent les facteurs à l’origine de l’incapacité prolongée et les modalités d'intervention auprès des personnes qui présentent un risque d'incapacité prolongée, ces modalités suscitant un intérêt évident. Les interventions multidisciplinaires, multimodales et ciblées – constituent les modalités les plus évoqués dans les études revues. En outre, l'étude des facteurs d'ordre psychosocial et ergonomique liés au milieu de travail, plus particulièrement chez les travailleurs tenus pour présenter un risque d'incapacité prolongée, est souvent considérée. La prise en compte du point de vue des différents intervenants, décideurs et praticiens constitue une tendance très répandue dans les études sur les interventions visant à prévenir l’incapacité prolongée chez les travailleurs indemnisés pour TMSOP. Enfin, la revue de littérature permet de dégager un certain nombre de stratégies susceptibles de favoriser la prévention de l'incapacité prolongée. Premièrement, le dépistage précoce des travailleurs blessés qui présentent un risque d'incapacité prolongée et la concentration des efforts d'intervention sur eux sont cités comme des moyens efficaces de prévenir l'incapacité prolongée. Deuxièmement, une intervention précoce fondée sur des approches multimodales incluant des facteurs biopsychosociaux et des facteurs du milieu de travail semble essentielle à un RAT efficace et durable, de même qu'à une gestion efficace des travailleurs exposés à un risque d'incapacité prolongée. Troisièmement, l'amélioration de la communication entre les décideurs et les intervenants et l'amélioration de la coordination entre les actions des acteurs impliqués dans le processus d'intervention sont présentées comme des stratégies étant essentielles pour accroître l'efficacité des interventions multidisciplinaires dans la prévention de l’incapacité prolongée. Les conclusions de la présente revue de la littérature élargissent les horizons des décideurs, des intervenants et des praticiens actifs dans le domaine de la réadaptation et du retour au travail, en leur fournissant des stratégies susceptibles de les guider dans la prévention de l’incapacité prolongée et de l'information sur des domaines précis qui nécessitent davantage de recherches concernant les travailleurs indemnisés pour TMSOP. Abstract Musculoskeletal disorders are among the most common types of work injury in industrialized countries and constitute a major economic burden on contemporary society. Moreover, workrelated musculoskeletal disorders (WRMSDs) are among the leading causes of significant human suffering, loss of productivity, and heavy financial costs to society. A large portion of this burden is associated with the number of workdays lost by a small percentage of workers being compensated for prolonged disability caused by musculoskeletal disorders. In the work disability research field over the past two decades, numerous studies have described and evaluated different types of interventions found to be helpful in preventing disability in workers who have WRMSDs, with many of these interventions originating from the rehabilitation and RTW fields. More recently, several systematic literature reviews and metaanalyses have been published with the aim of enlightening the decision-making process by providing evidence-based data on the best interventions for preventing work disability. However, very little of the literature addresses the issue of preventing prolonged disability in workers compensated for WRMSDs. In addition, the controversial nature of the results evaluating the effectiveness of the different interventions, the incomplete description of the participants involved in the studies, and the absence of explanations concerning the implementation process and strategies of the different programs and interventions make their application dangerous in actual practice. The endless proliferation of concepts in the work disability field, the diversity of implementation contexts and interventions, and the multiple interests and points of view of the stakeholders involved in the intervention process compound the complexity of the challenges faced, not only by researchers but also by managers and practitioners, in preventing prolonged disability. Many questions persist, particularly regarding the strategies to be used to prevent prolonged disability and promote successful rehabilitation and sustainable return-to-work (RTW). Stakeholders still need comprehensive reviews that provide a synthesis of and details on interventions and their application context, especially from the perspective of preventing prolonged disability. The main objective of this literature review was therefore to provide an overview of the main topics and trends in contemporary work disability research and to identify potentially successful intervention modalities for preventing prolonged disability in workers compensated for WRMSDs. A systematic electronic search of the literature was performed in ten scientific databases by a specialized librarian. The descriptors and key words searched in the databases pertained to five key concepts: 1) workplace absenteeism, 2) work-related musculoskeletal disorders, 3) work disability intervention, 4) rehabilitation and return-to-work intervention, and 5) compensation issues. After correcting the list of references obtained for duplications and misspellings and checking the references against the inclusion and exclusion criteria, 218 articles were selected for review: 186 articles reporting on one or more revelatory studies, 20 theoretical articles and 12 literature reviews. Descriptive quantitative analyses were performed on the 218 abstracts. In order to pinpoint the main topics and recent trends in research on intervention, the articles were first described in terms of the country of the first author, publishing journal, and type and stage of WRMSDs discussed. Second, the articles reporting on one or more revelatory studies (186) were categorized on the basis of their main objective, stakeholder(s) involved in the study or interested by the study’s results, research perspective, and area of application. A grid constructed especially for these purposes was used to categorize the articles and compile the relevant information. Two reviewers, whose decisions were made by consensus, proceeded to define the main categories and classes to be included in the grid, to categorize the articles, and to extract the pertinent information for describing the topics and trends. A database was created for further reference. Finally, qualitative analyses were performed on the articles found by the reviewers to describe strategic components of intervention aimed at preventing prolonged disability (88 out of the 186 articles that reported on one or more revelatory studies), to identify these strategic components in the different programs, policies, strategies, and guidelines reported by the authors, to identify the main strategies aimed at preventing prolonged disability, and, further, to explain their usefulness to practitioners and stakeholders in their decision-making processes. Five main topics were identified in the literature reviewed. The first major topic concerns the factors contributing to prolonged disability and a clear interest in developing modalities for screening people at risk of prolonged disability in order to provide them with appropriate interventions.
