Total diz replasmanı geçirmiş bireylerde geç dönem egzersiz eğitiminin etkinliğinin karşılaştırılması

Abstract
Bu çalışma, diz osteoartriti sonucu total diz replasmanı geçirmiş bireylere geç dönemde verilen egzersiz eğitiminin yaşam kalitesi, hareket korkusu, ağrı ve eklem sertliği üzerine olan etkisini incelemek amacıyla yapıldı. Çalışmaya en az 6 ay önce total diz replasmanı geçiren 55 yaş ve üzeri 14’ü kadın, 6’sı erkek 20 gönüllü birey dahil edildi. Bu çalışma için özel olarak egzersiz programı oluşturuldu. Bireylere uygulanacak 8 haftalık bu egzersiz programı haftada 5 gün, günde 2 defa 15’er tekrar olacak şekilde hastalara öğretildi ve hatırlatma amacıyla özel olarak hazırlanan egzersiz görsellerinin açıklamalarıyla birlikte yer aldığı broşürler bireylere verildi. Bireylerin egzersiz takibi düzenli olarak yapıldı. Egzersiz takibi için bireyler haftada 1 gün evlerinde gözlemlendi. Çalışmamızda yaşam kalitesini ölçmek için Short Form 36 (SF-36) ölçeği, eklem sertliğini değerlendirmek için Western Ontario Mc Masters Osteoartrit İndeksi (WOMAC) testi, ağrı için Visual Analog Skala (VAS) ölçeği ve hareket korkusunu ölçmek için ise Tampa Kinezyofobi Ölçeği (TKÖ) kullanıldı. Testler egzersiz öncesi ve sonrasında aynı fizyoterapist tarafından bireylere tekrarlandı. Toplanan veriler kaydedildi, analizleri yapıldı ve sonuçlar karşılaştırıldı. Elde edilen verilere bakıldığında; egzersiz öncesi ve sonrası ağrıda azalma olduğu görüldü. Aynı şekilde hareket korkusu ve eklem sertliğinde azalma olduğu, fiziksel fonksiyon, enerji ve ruh sağlığı parametresinde ise artma olduğu belirtildi. Bu değişimlerin anlamlı olduğu belirlendi (p<0,05). Fiziksel rol ve genel sağlık algısı parametrelerinde olumlu anlamda değişim olmasına rağmen sonuçlar anlamlı düzeyde değildi (p>0,05). Sonuç olarak total diz replasmanı geçiren hastalarda geç dönemde ağrıyı azaltmak, hareket korkusunu en aza indirgemek ve fiziksel fonksiyonu iyileştirmek için egzersiz tedavisinin kullanılabileceği gösterilmiş oldu.
