Abstract
Tutkimuksen tarkoituksena oli tehdä systemaattinen katsaus PLMT-oireyhtymästä. Katsaukseen kerättiin tietoa PLMT-oireyhtymän kliinisistä piirteistä, etiologiasta ja hoitomuodoista. Oireiston harvinaisuuden vuoksi informaatio perustuu tapausselostuksiin. \nTutkimusta varten haettiin Pubmed tietokannasta PLMT-oireyhtymään liittyviä tapausselostuksia tai tapaussarjoja, jotka täyttävät hakukriteerit. Lopullinen julkaisuiden määrä oli 96. Tapausten kokonaismäärä oli 187, jonka lisäksi tutkimuksessa käsitellään erillisenä 76 potilaan tapaussarja. \n \nPainful legs and moving toes (PLMT) on harvinainen liikehäiriö. Oireyhtymä on dokumentoitu ensimmäisen kerran vuonna 1976. Se ilmenee kipuna yhdessä tai useammassa raajassa ja siihen liittyy toistuvia, tahdosta riippumattomia varpaiden tai sormien liikkeitä. Oireyhtymästä on raportoitu myös harvinaisempi, kivuton muoto Painless legs and moving toes (PoLMT). Kivun vaikeusaste vaihtelee potilaiden välillä. Potilaan kokevat kivun yleensä häiritsevämpänä oireena kuin liikehäiriön. Oireyhtymän patogeneesi on tuntematon, eikä siihen ole löydetty täsmällistä hoitoa. PLMT:n erotusdiagnostisia vaihtoehtoja ovat esimerkiksi perifeerinen neuropatia, verenkiertohäiriöt, levottomat jalat -oireyhtymä tai Parkinsonin tauti. \n \nTutkimuksen perusteella suurimmalla osalla potilaista kipu liittyy oireyhtymään. Oireet painottuvat useammin alaraajoihin. Oireyhtymä on yleisempi naisilla kuin miehillä. Oireet alkavat useimmiten yli 50 vuoden iässä. Yleisimmät tunnistetut syyt oireyhtymän taustalla ovat perifeeriseen hermostoon vaikuttavia sairauksia, kuten perifeerinen neuropatia, radikulopatia tai perifeerisen hermon vaurio. Lähes puolella potilaista oireyhtymän taustalla oleva syy jää kuitenkin tuntemattomaksi. Lääkkeellisistä hoitomuodoista botuliinitoksiini-injektioilla on saatu paras hoitovaste. Tämän lisäksi yleisimmät lääkeaineet oireyhtymän hoidossa ovat olleet gapapentinoidit, klonatsepaami, amitriptyliini ja baklofeeni.
