La maladie parodontale est un processus inflammatoire qui se caractérise par la destruction des tissus de support des dents. Pour traiter cette maladie, un détartrage et un surfaçage radiculaire sont couramment utilisés. Le but de ce traitement est d'éliminer l'inflammation en retirant le biofilm bactérien, le tartre, le cément et la dentine infectés tout en débridant les surfaces radiculaires rugueuses pour promouvoir une nouvelle attache parodontale. Ce type de guérison devrait entraîner une diminution de l'inflammation gingivale causant une diminution de la profondeur de poche, une augmentation de la récession et un gain d'attache clinique. Afin de maximiser la guérison de cette thérapie, plusieurs adjuvants ont été utilisés dans le passé. Dans ce projet de maîtrise, nous nous attarderons sur l'utilisation de la « fibrine riche en plaquettes et leucocytes » (L-PRF) comme additif à la thérapie initiale dans le but d'améliorer son résultat. Afin de valider cette hypothèse, une étude de type contralatérale randomisée contrôlée à double insu a été utilisée pour vérifier la problématique. 8 sujets en bonne santé, non-fumeurs et souffrant d'une parodontite, ont reçu un traitement de surfaçage avec et sans L-PRF appliqué à l'intérieur de la poche parodontale. Par la suite, les sujets ont été suivis pendant 8-13 mois à la clinique de parodontie au 2e cycle de l'Université Laval afin d'évaluer la profondeur de poche, la récession gingivale ainsi que le niveau d'attache clinique des dents traitées. Les résultats comparatifs entre les deux traitements obtenus dans les conditions de notre étude démontrent qu'il n'existe aucune différence, peu importe la modalité de traitement choisi au niveau de la diminution de la profondeur de poche, de la récession ou du niveau d'attache clinique. En conclusion, les deux options de traitements offrent une alternative clinique adéquate. Par contre, il serait possible que le L-PRF apporte un bénéfice qui n'a pas été évalué dans cette étude.
