第一部分 不同心脏保存液的供体心脏保护效果的网状Meta分析<br />目的:心脏移植中,将供体心脏浸泡于4℃心脏保存液中。心脏保存液对供体心脏的保护效果直接影响着患者的生存率与生活质量。临床实践中最常用的三种心脏保存液有HTK液(Histidine-Tryptophan-Ketoglutarate solution)、Celsior液及UW液(University of Wisconsin solution),但何种保存液保护效果最佳尚无定论。本研究部分通过对HTK液、Celsior液及UW液进行网状Meta分析,评价3种心脏保存液的临床效果。方法:检索电子文献数据库PubMed、Embase、the Cochrane Library、the Centre for Evidence in Transplantation、the International Clinical Trials Registry Platform。所有电子文献数据库从建立日期检索至2019年1月17日。纳入患者年龄大于18岁、心脏移植患者及供体心脏使用HTK液、Celsior液及UW液保存的临床研究。主要终点指标是患者心脏移植术后30天生存率,次要终点指标是患者心脏移植术后原发性移植物功能障碍发生率。利用Newcastle-Ottawa Scale(NOS)评分量表对纳入的研究进行研究质量评价。传统Meta分析采用RevMan5.3软件,网状Meta分析采用GeMTC软件。<br />结果:共有8篇研究文献纳入,其中2篇比较HTK液与Celsior液,2篇比较HTK液与UW液,4篇比较Celsior液与UW液。应用UW液保存供体心脏移植后,患者的30天生存率高于HTK液与Celsior液(OR = 0.55, 95%CI: 0.37 - 0.69, P < 0.0001;OR = 0.72, 95%CI: 0.54 - 0.94, P = 0.02),但HTK液与Celsior液之间无差异。对于心脏移植术后30天生存率,网状Meta分析共纳入患者共计6658名,其中HTK组有467名患者,Celsior组有2098名患者,UW组有4093名患者。UW液排序第一的概率为95%,Celsior液排序第二的概率为85%,HTK液排序第三的概率为90%。应用UW液保存供体心脏移植后,患者的心脏移植术后原发性移植物功能障碍发生率低于Celsior液(OR = 1.34, 95%CI: 1.10 - 1.62, P = 0.003)。对于术后原发性移植物功能障碍发生率,网状Meta分析共纳入患者共计5600名,其中HTK组有244名患者,Celsior组有2067名患者,UW组有3289名患者。HTK液排序第一的概率为96%,Celsior液排序第二的概率为89%,UW液排序第三的概率为92%。结论:心脏移植患者应用UW液保存的30天生存率高于Celsior液与HTK液,并且原发性移植物功能障碍发生率低于Celsior液与HTK液。Celsior液与HTK液对比发现,二者之间供体心脏效果无差异。网状Meta分析提示,UW液供体心脏保存效果最佳,Celsior液的保存效果可能优于HTK液。第二部分 不同心脏保存液的供体心脏保护效果的iTRAQ定量蛋白质组学研究.目的:心脏保存液主要由不同离子、非渗透性物质、能量代谢底物、缓冲物质及抗氧化物等成分构成,其临床应用效果不同,尚未有定论何种心脏保存液效果最佳。蛋白质组学可以分析发现特定条件下蛋白质整体的变化水平。本研究部分通过iTRAQ定量蛋白质组学技术,评价UW液、Celsior液及HTK液保存后,心肌细胞内整体蛋白质数量与功能的变化。方法:选取Sprague-Dawley(SD)成年雄性大鼠12只,分为假手术组、HTK组、Celsior组及UW组。分别灌注不同组别对应心脏保存液,保存供体心脏6小时取材。各组提取蛋白质,进行iTRAQ标记与质谱鉴定。对鉴定出差异蛋白质筛选,分别进行GO功能注释、GO功能显著性富集分析、KEGG功能注释及KEGG Pathway显著性富集。结果:共鉴定到1804个蛋白质,进一步进行差异分析,3种保存液保存后差异蛋白变化总数相近,其中UW液差异蛋白总数最低,HTK液差异蛋白总数最高,Celsior液居中,并且差异蛋白均以蛋白下调变化为主。GO功能注释分析发现,相比于对照组,3种保存液差异蛋白主要涉及细胞(cell)、细胞区域部分(cell part)、细胞器(organelle)、结合(binding)、催化活性(catalytic activity)、结构分子活性(structural molecule activity)、细胞过程(cellular process)、单组织过程(single-organism process)及代谢过程(metabolic process)。GO富集分析,相比于对照组,HTK液与Celsior液上调差异蛋白与氧运输活性(oxygen transporter activity)、氧结合(oxygen binding)及血红素结合(heme binding)相关。HTK液下调差异蛋白与组蛋白赖氨酸甲基化(histone lysine methylation)及组蛋白H3-K4甲基化(histone H3-K4 methylation)。Celsior液下调差异蛋白与核小体结构、定位及功能相关。UW液下调差异蛋白与肽链内切酶抑制活性(endopeptidase inhibitor activity)、肽酶抑制活性(peptidase inhibitor activity)及肽链内切酶调节活性(endopeptidase regulator activity)相关。KEGG功能注释与Pathway富集分析,相比于对照组,HTK液、Celsior液及UW液下调差异蛋白分别与蛋白质消化吸收(Protein digestion and absorption)、糖尿病并发症AGE-RAGE信号通路(AGE-RAGE signaling pathway in diabetic complications)及补体与凝血级联(Complement and coagulation cascades)相关。结论:3种心脏保存液差异蛋白主要涉及细胞内核小体与蛋白酶体系统水平改变。HTK液与Celsior液下调差异蛋白涉及I型与III型胶原,UW液下调蛋白涉及补体系统。相比于UW液,HTK液与Celsior液上调差异蛋白均可能执行补体与凝血级联(Complement and coagulation cascades),并且Celsior液上调差异蛋白与HIF-1信号通路(HIF-1 signaling pathway)存在相关性。第三部分 冷诱导RNA结合蛋白在供体心脏保存中的iTRAQ定量蛋白质组学研究。目的:除心脏保存液添加各种成分达到保护效果外,维持低温也是供体心脏保护的核心要点之一,但其相关的分子机制并不明确。在冷刺激状态下,冷诱导RNA结合蛋白(Cold inducible RNA-binding protein, CIRBP)表达升高,参与细胞的各种生理过程,发挥着重要作用。本研究内容评价在供体心脏保存过程中,CIRBP涉及的相关分子机制。方法:选取SD、Cirbp-/-及Cirbp-Tg成年雄性大鼠各6只,分为假手术组、wild-type组、Cirbp-/-组及pCMV-Cirbp组。使用UW液保存供体心脏6小时取材。各组提取蛋白质,进行iTRAQ标记与质谱鉴定。对鉴定出差异蛋白质筛选,分别进行GO功能注释、GO功能显著性富集分析、KEGG功能注释及KEGG Pathway显著性富集。结果:共鉴定得到2192种蛋白质,差异分析发现,UW液保存后差异蛋白以下调蛋白为主,相比于wild-type组,Cirbp-/-组及pCMV-Cirbp组差异蛋白变化数量较多,但差异蛋白均以下调为主。GO功能注释分析,相比于wild-type组,Cirbp-/-组及pCMV-Cirbp组差异蛋白主要涉及细胞(cell)、细胞区域部分(cell part)、细胞器(organelle)、结合(binding)、催化活性(catalytic activity)、结构分子活性(structural molecule activity)、细胞过程(cellular process)、单组织过程(single-organism process)及代谢过程(metabolic process)。GO富集分析,Cirbp-/-组下调差异蛋白富集得到与核小体结构、定位及功能相关,pCMV-Cirbp组下调差异蛋白主要富集得到血液微粒(blood microparticle)。KEGG功能注释分析,相比于wild-type组,Cirbp-/-组上调差异蛋白与糖酵解与糖异生(Glycolysis/Gluconeogenesis)相关,pCMV-Cirbp组下调差异蛋白与氧化磷酸化(Oxidative phosphorylation)相关。KEGG Pathway富集分析,Cirbp-/-组上调差异蛋白有组氨酸代谢(Histidine metabolism)、甘氨酸、丝氨酸与苏氨酸代谢(Glycine, serine and threonine metabolism)、色氨酸代谢(Tryptophan metabolism)、精氨酸与脯氨酸代谢(Arginine and proline metabolism)及丙酮酸代谢(Pyruvate metabolism)相关,pCMV-Cirbp组下调差异蛋白与半乳糖代谢(Galactose metabolism)与氨基酸糖与核苷酸糖代谢(Amino sugar and nucleotide sugar metabolism)相关。结论:在供体心脏保存过程中,CIRBP可能参与DNA损伤修复过程、维持细胞内能量稳态及降低活性氧自由基,并且当其过表达时可能降低心肌细胞的应激刺激。
