Prise en charge de l’extravasation cytotoxique : tutoriels d’apprentissage par simulation pour améliorer la qualité des soins et la sécurité des patients sous chimiothérapie

Abstract
Le présent article traite d’un projet d’amélioration de la qualité mené en collaboration par une infirmière clinicienne coordonnatrice en oncologie et une infirmière enseignante spécialisée en simulations. Le projet a utilisé des tutoriels d’apprentissage progressif par simulation pour former les infirmières en oncologie travaillant dans un hôpital de soins tertiaires et évaluer leur auto-efficacité. L’extravasation cytotoxique a été étudiée dans une optique générale à l’aide d’un diagramme d’Ishikawa représentant les facteurs à l’origine de problèmes courants dans l’administration de la chimiothérapie et la pratique infirmière en oncologie. L’approche du cycle rapide d’amélioration (parfois rendu par l’acronyme PREA pour Planifier-Réaliser-Examiner-Agir) a été utilisée au moment de mettre en application la méthodologie du projet d’amélioration de la qualité et d’élaborer la stratégie de formation des infirmières en oncologie. Quant à eux, les tutoriels d’apprentissage par simulation constituaient une activité pédagogique innovante. Toutes les infirmières en oncologie ont été formées, le but étant d’améliorer leur efficacité à détecter et à prendre en charge l’extravasation. L’approche innovante offerte par les tutoriels par simulation a permis d’améliorer les compétences. Elle a également mené à une réflexion sur la formation continue et donné l’occasion d’évaluer les infirmières, deux éléments qui influenceront les pratiques cliniques en oncologie et permettront d’éviter aux patients certains effets indésirables de la chimiothérapie.
