Діабетична Невропатія (ДН) представляє собою серйозне ускладнення Цукрового Діабету (ЦД), яке викликає різноманітні прояви, що включають розлади чутливості, рухової функції та вегетативних процесів. За даними джерел, ДН поширена серед половини хворих на ЦД. ДН поділяється на симетричну (дистальна сенсомоторна поліневропатія, поліневропатія малих волокон, вегетативна невропатія) та асиметричну (мононевропатії та радикулоплексопатії). Дистальна сенсомоторна ДПН найпоширеніша, яка становить [75–90] % усіх невропатій. Більше чверті пацієнтів із ДН стикаються з больовим синдромом, що значно погіршує якість життя. Патогенез захворювання складний та зумовлений гіперглікемією, тривалістю діабету, курінням, ожирінням та іншими факторами. Розвиток починається з асимптоматичної поліневропатії, переходячи до чутливих розладів та порушень рухової функції. Адипокіни, такі як адипонектин та лептин, впливають на розвиток ДН. Взаємозв’язок із запальними цитокінами, такими як TNF-alpha, є важливим аспектом патогенезу. Для діагностики використовують шкали визначення ймовірності ускладнень, електронейроміографію та мікроскопічне обстеження. Sudoscan та Neuropad дозволяють неінвазивно визначати судомоторну дисфункцію. Біомаркери, такі як цитокіни та нейротрофічні фактори, є перспективним напрямком дослідження. Інтегрований підхід до дослідження відкриває можливості для розробки специфічних методів діагностики та лікування ДН, сприяючи покращенню якості життя пацієнтів, які стикаються з цією складною проблемою. Archived: https://doi.org/10.5281/zenodo.16779213 Abstact The article is devoted to the analysis of modern literature sources in the direction of highlighting the role of inflammation in the formation of diabetic polyneuropathy and emphasizing the prospects for further research to optimize the diagnosis, treatment and prognosis of this disease. Diabetic Neuropathy (DN) is a serious complication of Diabetes Mellitus (DM) that causes a variety of manifestations, including disorders of sensitivity, motor function, and autonomic processes. According to sources, DN is common among half of people with DM. DN is divided into symmetric (distal sensorimotor polyneuropathy, small-fibre polyneuropathy, autonomic neuropathy) and asymmetric (mononeuropathy and radiculoplexopathy). Distal sensorimotor diabetic polyneuropathy is the most common, accounting for [75–90]% of all neuropathies. More than a quarter of patients with DN experience pain syndrome, which significantly worsens their quality of life. The pathogenesis of the disease is complex and is caused by hyperglycemia, long-term diabetes, smoking, obesity, and other factors. The development begins with asymptomatic polyneuropathy, progressing to sensory disorders and motor dysfunction. Adipokines, such as adiponectin and leptin, influence the development of DN. The relationship with inflammatory cytokines, such as TNF-alpha, is an important aspect of pathogenesis. Scales (aimed at determining the presence of this complication with varying degrees of probability), electroneuromyography, and microscopic examination are used for diagnosis. Sudoscan and Neuropad allow non-invasive detection of motor dysfunction. Biomarkers such as cytokines and neurotrophic factors are a promising area of research. An integrated research approach opens up opportunities for the development of specific methods for the diagnosis and treatment of DN, contributing to the improvement of the quality of life of patients facing this complex problem.
