Abstract
BECAUSE OF NEISSERIA GONOR-rhoeae resistance to all priorfirst-line antimicrobial agents,cephalosporin therapywith ad-juvant azithromycin or doxycycline is recommended for treatment of gonor-rhea.1-5 Cefixime is the only oral ceph-alosporin recommended for gonor-rhea treatment, critical to the success of expedited partner therapy. An in-crease in theminimum inhibitory con-centration (MIC) of N gonorrhoeae to cefixime, and to a lesser extent, an in-tramuscularly administered cephalo-sporin, ceftriaxone, has been identi-fied in cultured isolates worldwide.6-8 The World Health Organization has sounded alarms for the threat of un-treatable gonorrhea.9 The US Centers for Disease Control and Prevention (CDC) recently re-vised recommendations for treatment of uncomplicated urogenital, anorec-tal, and pharyngeal gonorrhea.4 Ceftri-axone, 250 mg, intramuscularly com-bined with either azithromycin, 1 g, orally or doxycycline, 100 mg, orally twice a day for 7 days is the sole pre-ferred treatment regimen, with oral cephalosporins no longer recom-mended as front-line therapy. If cefix-ime is used as an alternative therapy for gonorrhea, it is to be followed with a test of cure in 1 week. Use of cefiximeSee also p 185 and Patient Page.
