Réanimation cardiopulmonaire des arrêts cardiorespiratoires intra hospitaliers au Nord-Kivu : évaluation du niveau de connaissance et perception des soignants et leurs déterminants

Introduction : L’Arrêt Cardiorespiratoire Intra hospitalier (ACRIH) est un problème de santé publique insuffisamment documenté en Afrique francophone. Cette étude évalue le niveau de connaissance des soignants de la province du Nord-Kivu en réanimation des ACRIH. Matériel et méthodes : Il s’agit d’une étude transversale, quantitative et analytique menée d’août à octobre 2023 chez 203 soignants congolais de l’hôpital HEAL Africa et de l’Hôpital Général de Référence de Kyondo. Un questionnaire leur a été administré. La corrélation entre variables a été analysée par le test Chi carré et la régression logistique au seuil de signification de 5%. Résultats : L’âge moyen des soignants était de 35,1ans (±10,6ans) dont 52,2% étaient de sexe masculin. La majorité était mariée (57,6%) et prestait à l’Hôpital HEAL Africa (60,1%). Les infirmiers étaient majoritaires (42,3%). Le niveau de connaissance en Réanimation Cardiopulmonaire (RCP) est globalement médiocre ; 57,1% des soignants ont obtenu une cote comprise entre 0-30%, une moyenne de 30% (±17,6) et seulement 5,9% ont obtenu une cote ≥ à 50%. 65% des soignants estiment que la RCP est fréquente, 87,7% l’estiment utile et 53,7% ne font aucun suivi post arrêt cardiorespiratoire. 82,2% des soignants n’utilisent pas le score d’alerte d’urgence. Après régression logistique, le seul déterminant de la réussite au test d’évaluation de connaissance en RCP a été la connaissance d’au moins 6 causes réversibles de l’Arrêt Cardiorespiratoire (ACR). Conclusion : La plupart des soignants de la province du Nord-Kivu ont une connaissance médiocre en RCP de qualité des ACRIH. Elle reconnait l’utilité de la RCP et la pratique fréquemment. Le seul déterminant du niveau de connaissance en RCP est la connaissance d’au moins six causes réversibles de l’ACR. Une formation structurée en RCP des ACRIH est indispensable pour améliorer leur niveau de connaissances.
