Abstract
Objectif Les lignes directrices actuelles recommandent d’amorcer le traitement des hypoglycémies non sévères (NS) avec 15g de glucides à absorption rapide (CHO) à intervalles de 15 minutes lorsque la glycémie veineuse descend <4,0 mmol/L (70 mg/dL) Malgré cette recommandation, le traitement de l’hypoglycémie NS demeure un défi important pour les personnes vivant avec le diabète de type 1 (PVDT1). L’objectif de cette étude vise à évaluer l’efficacité de 15g de CHO lorsque consommé à un seuil glycémique plus élevé. Méthode Un total de 29 PVDT1 ont participé à cette étude ouverte en chassé-croisé. Après avoir administré, en milieu de soins, un bolus d’insuline sous-cutané en laboratoire pour induire une descente de la glycémie veineuse, 16g de CHO oraux ont été administrés à une glycémie plasmatique (GP) de <4,0 mmol/L (70 mg/dL), ≤4,5 mmol/L (80 mg/dL mmol/L) ou ≤5,0 mmol/L (90 mg/dL). L’issue primaire est le temps passé en hypoglycémie (<4,0 mmol/L) après le traitement initial. Résultats Lorsqu’on compare le groupe de traitement à <4,0 mmol/L (contrôle) aux groupes de ≤4,5 mmol/L et ≤5,0 mmol/L, 100% vs 86% (p=0,12) vs 34% (p<0,0001) des participants ont atteint l’hypoglycémie. Ces évènements d’hypoglycémie ont duré 26,0±12,6 vs 17,9±14,7 (p=0,02) vs 7,1±11,8 (p=0,02) minutes, avec un nadir glycémique de 56,6±9,9 mg/dL (3,1±0,9 mmol/L) vs 63,6±7,9 mg/dL (3,5±0,4 mmol/L) (p=0,008) vs 73,5±9,4 mg/dL (4,1±0,5 mmol/L) (p=0,002) respectivement. Dans le groupe contrôle, 69% des participants ont nécessité plus d’un traitement pour atteindre ou maintenir la normoglycémie (≥4,0 mmol/L) comparativement à 52% dans le groupe ≤4,5 mmol/L et 31% dans le groupe ≤5,0 mmol/L, et ce, sans hyperglycémie rebond significative (>10,0 mmol/L ou >180 mg/dL) dans la première heure suivant le traitement. Conclusion Les PVDT1 pourraient bénéficier d’une correction avec des CHO oraux à un seuil de GP plus élevé pour certains épisodes d’hypoglycémie NS imminente.
