Abstract
Nous avons réalisé en mars 2014 les premiers cas de revascularisation chirurgicale du myocarde au service de chirurgie de l'ICA. L'objectif de notre présente étude est de rapporter les résultats de cette expérience inaugurale. Cette étude concernait trois (03) patients dont deux (02) hommes et une (01) femme d'Age moyen 51,7 ans (extrême 46 et 55 ans) insuffisant coronaire. Ces patients étaient symptomatiques avec un angor invalidant en classe III de la Canadian cardiac society (CCS). La coronarographie a mis en évidence des lésions tritronculaires dans deux cas et une sténose du tronc coronaire gauche dans un cas. Deux patients avaient un antécédent d'angioplastie coronaire. Tous les patients ont été opérés sous circulation extracorporelle et clampage aortique. Deux d'entre eux ont fait l'objet d'un double pontage et un a bénéficié d'un triple pontage. La mortalité opératoire et hospitalière était nulle. Au dernier contrôle, tous les patients étaient asymptomatiques avec une épreuve d'effort négative. Cette expérience permet de démontrer la faisabilité de la revascularisation chirurgicale du myocarde en toute sécurité dans notre institution avec des résultats satisfaisants.
