Abstract

目的明确血清同型半胱氨酸(homocysteine,Hcy)水平与血管性痴呆(vascular dementia,VaD)认知功能相关性。方法收集2015年7月至2016年6月在我院神经内科门诊及住院的VaD连续病例。同时收集我院体检中心年龄、性别及受教育程度相匹配的受试者进行对照研究。采用酶循环法检测血清Hcy水平。主要应用简易精神状态检查(mini-mental state examination,MMSE)、蒙特利尔认知评估(Montreal cognitive assessment,MoCA)进行认知功能检测。结果入组VaD患者76例,健康对照组48例。VaD组平均Hcy水平约26.18±7.86μmol/L,对照组平均水平约13.63±3.40μmol/L,两组Hcy水平差异具有显著性,P=0.00(P<0.05)。正常对照组MMSE为27.96±1.81分,VaD组为13.37±3.44分,二者有明显差异,P=0.00。通过直线回归分析,我们发现VaD患者MMSE得分与血清Hcy水平有较明显的负相关,R<sup>2</sup>=0.4123。正常对照组MoCA得分为27.56±1.69分,VaD组为11.17±3.54分,二者有明显差异,P=0.00。通过直线回归分析,VaD患者MoCA与血清Hcy水平有更明显的负相关性,R2=0.5089。结论 VaD与高Hcy血症有一定的相关性,且Hcy水平越高与认知功能损伤越重。
