PENGARUH REGIMEN KOMBINASI TRANSCUTANEOUSELECTRICAL NERVE STIMULATION (TENS) DAN INFRA REDRADIATION (IRR) DENGAN EXERCISE THERAPY TERHADAP NYERIDAN FUNGSIONAL LUTUT PADA PETANI PENDERITAOSTEOARTHRITIS: A QUASI EXPERIMENTAL STUDY

Abstract
Skor nyeri yang tinggi serta keterbatasan fungsional lutut yang rata-rata mengganggu sehingga akan mempengaruhi produktifitas petani. Berdasarkan fenomena ini, peneliti mendapatkan bahwa nyeri dan fungsional lutut petani penderita Osteoarthritis mempunyai nilai bervariasi. Sehingga peneliti ingin melakukan kajian lebih lanjut tentang pengaruh regimen kombinasi transcutaneous electrical nerve stimulation (TENS) dan infra red radiation (IRR) dengan exercise therapy terhadap nyeri dan fungsional lutut pada petani penderita Osteoarthritis. Tujuan penelitian ini adalah menganalisis TENS dan IRR dengan exercise therapy terhadap nyeri dan fungsional lutut pada petani penderita Osteoarthritis. Jenis penelitian ini quasi eksperimental menggunakan pre and post test two group design. Penelitian ini dilakukan di wilayah kerja Puskesmas Karang Pucung Lampung Selatan pada bulan Mei-Juni 2025. Populasi penelitian adalah petani penderita osteoatrritis sebanyak 68 orang dengan total sampling. Sebanyak 34 petani diberikan exercise therapy selama 25 menit dan 34 petani diberikan kombinasi TENS dan IRR selama 30 menit dilakukan 3 kali dalam seminggu selama 2 minggu. Pengukuran nyeri menggunakan Visual Analogue Scale (VAS) dan fungsional lutut menggunakan Western Ontario and Mcmaster Universities Osteoarthritis Index (WOMAC). Analisis menggunakan Wilcoxon test dan Mann Whitney test. Hasilnya mendapatkan perbedaan skor nyeri dan fungsional lutut sebelum dan sesudah diberikan exercise therapy pada petani. Ada perbedaan skor nyeri dan fungsional lutut sebelum dan sesudah diberikan kombinasi TENS dan IRR (8.00±2-5.00±3) (64.50±37-39.50±23) dibandingkan exercise therapy (7.50±2 -3.00±1) (66.00±47-22.00±4) p value 0.001 pada petani. Petani dianjurkan melakukan latihan peregangan secara rutin berupa latihan- latihan fisioterapi agar petani tetap aktif sehingga dapat mengurangi nyeri lutut, serta meningkatkan kemampuan bergerak saat bekerja di ladang atau kebun. Saran bagi Puskesmas Karang Pucung adalah menjalin kerjasama dengan pihak terkait untuk menyelenggarakan program edukasi dan penanganan OA pada petani menggunakan tindakan exercise therapy. This study aimed to analyze the effects of a combined regimen of Transcutaneous Electrical Nerve Stimulation (TENS) and Infrared Radiation (IRR) with exercise therapy on pain and knee function in farmers with OA. This quasi-experimental study used a pre- and post-test two- group design, conducted in May–June 2025 at the Karang Pucung Public Health Center, South Lampung. A total of 68 farmers with OA were selected through total sampling. Thirty-four participants received exercise therapy for 25 minutes, and the other 34 received a combination of TENS and IRR for 30 minutes, three times a week for two weeks. Pain was measured using the Visual Analogue Scale (VAS), and knee function was assessed using the Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC). Data were analyzed using the Wilcoxon and Mann-Whitney tests. The results showed significant differences in pain and knee function scores before and after both interventions. Exercise therapy showed greater improvement (pain score: 7.50±2 to 3.00±1; function score: 66.00±47 to 22.00±4) compared the combination of TENS and IRR (pain score: 8.00±2 to 5.00±3; function score: 64.50±37 to 39.50±23), with a p-value of 0.001. It is recommended that farmers perform regular physiotherapy-based stretching exercises to reduce pain and improve mobility. The health center is advised to collaborate with stakeholders in implementing educational and therapeutic programs for OA management among farmers.
