Background: il carcinoma mammario rappresenta la neoplasia più diffusa tra le donne e un importante ambito di prevenzione oncologica. L’attività fisica è riconosciuta come fattore modificabile potenzialmente protettivo, ma la letteratura presenta risultati eterogenei in relazione all’intensità, alla durata e al periodo di esposizione. Obiettivi: questa revisione sistematica ha valutato l’associazione tra i livelli di attività fisica e il rischio di carcinoma mammario, considerando differenze per stato menopausale e periodo di vita (giovinezza vs età adulta). Metodi: la ricerca è stata condotta nei database PubMed e Scopus fino a ottobre 2025, includendo studi di coorte prospettici in lingua inglese che riportassero misure quantitative di attività fisica in MET-h/settimana o in ore/settimana, e stime di rischio (HR o RR) per incidenza di carcinoma mammario. La qualità metodologica è stata valutata tramite la Newcastle–Ottawa Scale; la sintesi è qualitativa, data l’eterogeneità degli studi. Risultati: sono stati inclusi 20 studi, per un totale di oltre 1,15 milioni di partecipanti e follow-up medi compresi tra 6 e 22 anni. Diciassette studi su venti hanno mostrato un basso rischio di bias. L’attività fisica elevata è risultata associata a una riduzione del rischio di carcinoma mammario, più marcata nelle donne in post-menopausa (riduzioni tra il 9% e il 47%) rispetto a quelle in pre-menopausa, nelle quali l’effetto emerge oltre soglie di circa 27–30 MET-h/settimana. L’attività fisica in gioventù sembra avere un effetto protettivo anche in età adulta. Conclusioni: nonostante i limiti legati al disegno osservazionale, all’autovalutazione dell’attività fisica e all’eterogeneità delle misure, le evidenze supportano un effetto protettivo coerente dell’attività fisica nei confronti del carcinoma mammario. Promuovere stili di vita attivi costituisce quindi una strategia di prevenzione primaria efficace e una responsabilità etico-professionale per i professionisti sanitari.
