Middle cerebral artery stenosis-associated brain perfusion abnormalities: a study using arterial spin labeling magnetic resonance imaging

Contexto: A estenose da artéria cerebral média (ACM) é uma causa comum de acidente vascular cerebral isquémico. Assim, as repercussões hemodinâmicas de uma estenose da ACM podem ser alterações da perfusão cerebral, detetáveis mediante imagens de ressonância magnética (RM) usando marcação de spins arteriais (ASL). Deste modo, hipotetizamos que sujeitos com estenose única, unilateral e presumivelmente aterosclerótica da ACM apresentariam alterações da perfusão cerebral ipsilaterais, tais como hipoperfusão e artefactos de trânsito arterial (ATA), especialmente em contexto de grau de estenose > 70% (estenose grave). Métodos: Foram incluídos dezoito sujeitos, assintomáticos, com estenose da ACM. Onze (61%) tinham estenose da ACM esquerda (sete dos quais apresentavam estenose grave) e sete (39%) tinham estenose da ACM direita (quatro dos quais apresentavam estenose grave). Os autores avaliaram, visualmente, a hipoperfusão e os ATA, tendo utilizado o software ExploreASL, para quantificar o fluxo sanguíneo cerebral (CBF) nos territórios da ACM esquerda e direita. Resultados: As imagens ASL revelaram ATA, ipsilaterais à estenose, em oito (44%) sujeitos e regiões de hipoperfusão cerebral, ipsilaterais à estenose, em cinco (28%) sujeitos. Basicamente, 73% dos sujeitos com estenose grave apresentavam ATA ipsilaterais e 46% dos sujeitos com estenose grave apresentavam regiões de hipoperfusão cerebral ipsilaterais. Usando a escala Alberta Stroke Programme Early Computed Tomography Score, a presença de ATA, em todas as regiões da ACM, associava-se quer com a ocorrência, quer com a gravidade da estenose ipsilateral (p < 0,05). A hipoperfusão detetada visualmente, em território da ACM, associava-se com a gravidade da estenose ipsilateral (p < 0,05), mas não se detetou uma diminuição significativa do CBF, relativamente ao território da ACM contralateral. Não foram encontradas associações, estatisticamente significativas, entre os valores de CBF e a ocorrência de estenose, nem entre os referidos valores e a gravidade da estenose. Conclusão: A presença de ATA ipsilaterais a uma estenose, em todas as regiões da ACM afetada, foi a alteração da perfusão cerebral mais relevante encontrada nas imagens de RM ASL.
