Introdução | A osteonecrose maxilar induzida por medicamentos (MRONJ) é uma complicação associada ao tratamento oncológico. Inicialmente relacionada ao uso de bifosfonatos, também se associa ao uso de outros fármacos, como os inibidores de RANKL e terapias-alvo. Este aumento acompanha a utilização crescente de terapias farmacológicas avançadas e a sua complexidade, gerando um crescente interesse científico sobre a condição. Objetivos | Avaliar a prevalência de MRONJ em pacientes oncológicos, identificar os fatores de risco associados à sua ocorrência e explorar as implicações clínicas da doença. Analisar a relação entre os fármacos oncológicos e o desenvolvimento de MRONJ, com foco nas mais recentes descobertas científicas. Metodologia | Pesquisa bibliográfica nas bases de dados PubMed-Medline, Scopus, Cochrane Database of Systematic Reviews, Scielo e EMBASE, entre 2010 e Março 2025. Os estudos descrevem a MRONJ em pacientes oncológicos tratados com terapêuticas farmacológicas, com ênfase em artigos primários e ensaios clínicos randomizados. Foram seguidas as diretrizes do PRISMA e a qualidade metodológica dos estudos foi avaliada através da escala Newcastle-Ottawa. Resultados | A MRONJ é predominantemente associada ao uso de bifosfonatos e inibidores de RANKL, representando mais de 90% dos casos. Novos medicamentos como lenalidomida, docetaxel e terapias-alvo foram identificados como agentes causadores. A prevalência da MRONJ em pacientes oncológicos varia com a terapêutica utilizada, sobretudo nos pacientes com cancro da mama, devido ao uso de agentes anti-reabsortivos. Os principais fatores de risco incluem idade avançada, sexo feminino, tratamentos prolongados e combinação de terapias. A MRONJ compromete significativamente a saúde oral, qualidade de vida e alimentação e fala. Conclusão | A MRONJ é uma complicação importante do tratamento oncológico. Embora os avanços nas terapias farmacológicas tenham aumentado a eficácia dos tratamentos, o risco de MRONJ permanece elevado, principalmente devido ao uso de agentes anti-reabsortivos e terapias-alvo. A monitorização rigorosa dos pacientes, a implementação de estratégias preventivas e a deteção precoce da condição minimizam os impactos da MRONJ. A investigação sobre tratamentos eficazes e a gestão desta complicação são áreas prioritárias para melhorar a qualidade de vida dos pacientes.
