Abstract
Mục tiêu: Tổng hợp bằng chứng về tác dụng điều hòa miễn dịch của sâm Ngọc Linh (Panax vietnamensis) và nhân sâm Hàn Quốc (Panax ginseng), so sánh thành phần đặc trưng, cơ chế tác động và dữ liệu lâm sàng, từ đó gợi ý ứng dụng thực hành. Phương pháp: Tìm kiếm PubMed/PMC, Journal of Ginseng Research, các bản thẩm định/đơn dược liệu (EMA/HMPC, Health Canada) và tổng quan gần đây đến 17/10/2025. Ưu tiên tổng quan, khuyến cáo/đơn dược liệu, thử nghiệm người về chỉ số miễn dịch và nghiên cứu tiền lâm sàng chủ chốt về saponin đặc trưng (ví dụ majonoside‑R2 của P.vietnamensis; Rg1/Rb1/Rg3/Re của P.ginseng). Tổng hợp tường thuật có cấu trúc. Kết quả: Cả hai loài đều có tác dụng điều hòa miễn dịch trên cả miễn dịch bẩm sinh và thích nghi thông qua ginsenosides/saponin, ảnh hưởng chức năng đại thực bào, NK, tế bào tua và định hướng biệt hóa T, cùng điều chỉnh các cytokine (TNF‑α, IL‑6, IL‑1β, IFN‑γ). P.vietnamensis nổi bật với nhóm saponin ocotillol (đặc biệt majonoside‑R2/vina‑ginsenoside R2) thể hiện tác dụng kháng viêm và định hướng Th17 trong tiền lâm sàng; dữ liệu lâm sàng còn hạn chế. Với P.ginseng, có nhiều bằng chứng trên người về tăng đáp ứng vaccine và điều hòa miễn dịch, song không đồng nhất về chế phẩm/liều lượng. Tính an toàn nhìn chung tốt; xác thực chất lượng rất quan trọng do nguy cơ pha tạp/giả mạo. Kết luận: Bằng chứng ủng hộ lợi ích miễn dịch của cả hai; P.ginseng có dữ liệu người mạnh hơn, trong khi P.vietnamensis cho thấy tiềm năng cơ chế khác biệt. Cần các nghiên cứu đối đầu, chuẩn hóa chế phẩm và xác định liều‑thời gian tối ưu.
