Abstract
A mucormicose é a 3ª causa mais frequente de infecção fúngica invasiva, após a candidíase e a aspergilose, com uma incidência de 1,5% no transplante pulmonar. A suspeita inicial baseia-se nos sinais e sintomas apresentados, que variam conforme a forma da doença: 1-rinocerebral, 2- pulmonar, 3- cutânea, 4- gastrointestinal e 5- disseminada. A presença de úlcera necrótica com escara enegrecida (cutânea ou mucosa) é uma lesão característica e deve ser considerada um sinal de alerta em pacientes de risco. A tomografia computadorizada e ressonância magnética são utilizadas para identificar lesões e avaliar a extensão do acometimento. A histopatologia e cultura são essenciais para o diagnóstico. No caso relatado, o tratamento consistiu de desbridamento cirúrgico extenso e administração de antifúngicos, incluindo anfotericina B tópica e anfotericina B lipossomal intravenosa. Posaconazol VO também foi associado. Curativos VAC (Vacuum Assisted Closure) nas áreas desbridadas foram realizados bem como oxigenioterapia hiperbárica. O desfecho foi desfavorável.
