Maternal Sleep-Disordered Breathing in Pregnancy and Relationship with Child Head Circumference

Abstract
Rationale: Maternal sleep disordered breathing (SDB) increases in prevalence as pregnancy progresses. As a sub-study of the 3D pregnancy and birth cohort in Quebec, we have previously shown that maternal SDB in pregnancy is associated with an increased risk of delivering small for gestational age infants. However, it is not yet clear whether exposure to SDB in pregnancy alters head circumference growth trajectories among children.Methods:A retrospective, longitudinal analysis of the 3D Cohort and Transition Study (a follow-up study of the 3D Cohort assessing child mental health and school readiness; PI: J. Séguin) was performed in a subsample of children who had: 1) mothers with polysomnography during third trimester 2) at least one head circumference measurement performed from birth to 5 years of age. Maternal SDB was defined by an apnea-hypopnea index (AHI) ≥ 10 from third trimester polysomnography. Head circumference for offspring was measured using standardized procedures at birth, 3m, 6m, 12m, 24 m and 5 years of age. Linear regression and mixed models were developed adjusting for maternal pre-pregnancy body mass index (BMI), gestational age, maternal age and maternal education. Results:Based on our inclusion criteria, 222 children had available data. Maternal characteristics were: 82 % were White, mean (SD) age was 31(4.3) and pre-pregnancy BMI was 23(4) kg/m². Head circumference growth trajectories in boys and girls showed that there was a slight decrease in head circumference growth in boys exposed to SDB during pregnancy. However, adjusted linear regression analysis assessing the relationship between head circumference and AHI at each time point and adjusted cubic mixed model assessing for differences in head circumference trajectories were not significant in SDB vs no SDB groups. ConclusionsIn this study, we showed that SDB exposure during pregnancy was not associated with head circumference growth trajectories assessed from birth until 5 years of age
