Abstract
Background and aims: Over-the-counter analgesics, including non-steroidal anti-inflammatory drugs and corticosteroids like dexamethasone, are commonly used after injury.However, recent research suggests that their widespread use may suppress the inflammatory response in acute pain, inadvertently increasing the risk of developing chronic conditions.We hypothesized that using conditioned serum (CS), a controlled physiological activation of the immune response, could be a more effective therapeutic approach against chronic pain than inhibiting inflammation. Methods:We examined the effect of conditioned serum in two different animal pain models (the inflammatory model of plantar incision and the SNI model of neuropathy).Following the surgery, 6-8-week-old mice of both sexes received 0.5 mg/kg/day of the corticosteroid dexamethasone, or 1% DMSO subcutaneously for six consecutive days.Conditioned serum was prepared from human subjects following Orthogen protocol, it was then injected into the mice via different routes of administration at early or later time points post-surgery.Tactile allodynia was measured using Von Frey filaments at regular intervals post-surgery using Dixon's up-anddown method.Results: We found that dexamethasone substantially delayed recovery to the baseline after plantar incision surgery, such that the duration of the overall pain episode was significantly prolonged to 100 days instead of 20 days in saline-treated mice.Intraplantar and intraperitoneal injections of conditioned serum, at early or later time points, prevented dexamethasone-induced chronic pain following incisional paw surgery.We also found that CS did not produce analgesia in the SNI mice.également observé que le sérum conditionné n'induisait pas d'analgésie chez les souris ayant subi une neurotomie partielle du nerf sciatique (SNI).Conclusion: Nos résultats valident les observations précédemment rapportées selon lesquelles les traitements à la dexaméthasone entraînent une hyperalgésie durable dans un modèle de douleur post-chirurgicale plus cliniquement pertinent.Les injections de sérum conditionné se sont révélées efficaces pour prévenir la douleur persistante dans le modèle chronique postchirurgical induit par la dexaméthasone ; cependant, cet effet protecteur n'a pas été observé dans le modèle de neurotomie partielle du nerf sciatique (SNI).Ces résultats soulignent le rôle critique du système immunitaire dans la modulation de la douleur, suggérant que l'activation des voies immunitaires pourrait offrir une protection contre la transition de la douleur aiguë à la douleur chronique.
