Background: Chronic obstructive pulmonary disease (COPD) is a prevalent respiratory disease and a leading global cause of morbidity and mortality.COPD is frequently accompanied by cardiovascular comorbidities, including atrial fibrillation (AF), a common heart arrhythmia that significantly elevates the risk of stroke and mortality.Notably, current guidelines for COPD management recommend that oral anticoagulant (OAC) treatment decisions for stroke prevention in patients with concurrent AF be made independently of COPD status.However, it remains unclear how the incidence of AF and the prescribing patterns of OACs have evolved over time among patients with COPD.Understanding trends in AF incidence and OAC use is crucial to optimizing stroke prevention strategies in this high-risk population.Objectives: The two primary objectives of this manuscript-based thesis were to assess trends in the incidence of AF and subsequent rates of OAC initiation among patients with COPD in primary care in the UK from 2010 to 2022. Methods:We conducted a population-based cohort study using the UK Clinical Practice Research Datalink (CPRD), linked with the Hospital Episodes Statistics database.We assembled a cohort of patients aged 50 years and older with COPD and no history of AF prior to the diagnosis of COPD, who contributed data to the CPRD between January 1, 2010, and December 31, 2022.To assess rates of OAC initiation in the year following AF diagnosis, we formed the sub-cohort of patients diagnosed with AF after the diagnosis of COPD.Using a Poisson model, we estimated annual rates of incident AF and incidence rate ratios, adjusted for age and sex, comparing the rate in each calendar year to that in 2010 and to the rate in the year preceding each calendar year.We also stratified rates by age, sex, and markers of COPD severity.We repeated these analyses in the sub-cohort of patients with COPD and incident AF to estimate rates of OAC initiation overall, and separately for vitamin K antagonists (VKAs), direct oral anticoagulants (DOACs), and individual DOACs.Results: Among 444,668 patients with COPD in UK primary care during the study period, 37,645 (8.5%) were newly diagnosed with AF.AF incidence gradually increased over the study period, reaching a 29% higher rate in 2022 compared to 2010 [rate ratio (RR) 1.29; 95% confidence interval (CI) 1.07-1.56].Throughout the study period, AF incidence was higher in males, older individuals, and those with more severe COPD.Among the 37,645 patients with COPD newly diagnosed with AF, 23,094 (66.2%) were newly prescribed an OAC in the year following AF diagnosis.Over the study period, the rate of new OAC use prominently increased, reaching a 2.9fold higher rate in 2022 compared to 2010 (RR 2.91, 95% CI 2.33-3.65).New VKA use declined markedly, with a 96% lower rate in 2022 compared to 2010 (RR 0.04, 95% CI 0.02-0.10).Meanwhile, the rate of new DOAC use rose sharply from 2012 onwards, with a 64-fold higher rate in 2022 versus 2012 (RR 64.25, 95% CI 33.74-122.34).During the later years (2020-2022), apixaban was the most prescribed OAC (61.6% of all OACs), followed by edoxaban (22.1%), rivaroxaban (13.6%),VKAs (2.1%), and dabigatran (0.6%).OACs were most frequently prescribed to individuals aged 70-79 and to those with milder COPD.Conclusions: From 2010 to 2022, the incidence of AF and OAC initiation increased among patients with COPD in UK primary care.However, OAC use remained less than optimal among older individuals and those with more severe COPD.These trends highlight a potential treatment gap and the need for further research on stroke prevention strategies in patients with comorbid COPD and AF.iii Abrégé (français) Contexte: La maladie pulmonaire obstructive chronique (MPOC) est une maladie respiratoire répandue et l'une des principales causes de morbidité et de mortalité dans le monde.La MPOC s'accompagne souvent de comorbidités cardiovasculaires, notamment la fibrillation auriculaire (FA), une arythmie cardiaque courante qui augmente considérablement le risque d'accident vasculaire cérébral (AVC) et de mortalité.Les directives actuelles pour le traitement de la MPOC recommandent notamment que les décisions relatives au traitement par anticoagulants oraux (ACO) pour la prévention des AVC chez les patients atteints de FA concomitante soient prises indépendamment de l'état de la MPOC.Cependant, l'évolution de l'incidence de la FA et des schémas de prescription des ACO chez les patients atteints de MPOC n'est pas claire.Il est essentiel de comprendre les tendances de l'incidence de la FA et de l'utilisation des ACO pour optimiser les stratégies de prévention des AVC dans cette population à haut risque.Objectifs: Les deux objectifs principaux de cette thèse avec manuscrit étaient d'évaluer les tendances de l'incidence de la FA et les taux ultérieurs d'initiation d'ACO chez les patients atteints de MPOC dans les soins primaires au Royaume-Uni de 2010 à 2022.Méthodes: Nous avons mené une étude de cohorte basée sur la population en utilisant la base de données Clinical Practice Research Datalink (CPRD) du Royaume-Uni, liée à la base de données Hospital Episodes Statistics.Nous avons constitué une cohorte de patients âgés de 50 ans et plus, atteints de MPOC et sans antécédents de FA avant le diagnostic de MPOC, qui ont fourni des données au CPRD entre le 1er janvier 2010 et le 31 décembre 2022.Pour évaluer les taux d'initiation des ACO dans l'année suivant le diagnostic de FA, nous avons formé la sous-cohorte de patients atteints de MPOC chez qui une FA a été diagnostiquée après le diagnostic de MPOC.À l'aide d'un modèle de Poisson, nous avons estimé les taux annuels de FA incidente et les ratios de taux d'incidence, ajustés sur l'âge et le sexe, en comparant le taux de chaque année civile à celui de 2010 et à celui de l'année précédant chaque année civile.Nous avons également stratifié les taux en fonction de l'âge, du sexe et des marqueurs de gravité de la MPOC.Nous avons répété ces analyses dans la sous-cohorte des patients atteints de MPOC et de FA incidente pour estimer les taux d'initiation des ACO, et séparément des antagonistes de la vitamine K (AVK), des anticoagulants oraux directs (AOD), et des AOD individuels.Résultats : Parmi les 444 668 patients atteints de MPOC dans les soins primaires au Royaume-Uni au cours de la période d'étude, 37 645 (8,5%) ont été nouvellement diagnostiqués avec une FA.L'incidence de la FA a progressivement augmenté au cours de la période d'étude, atteignant un taux 29% plus élevé en 2022 qu'en 2010 [rapport de taux (RR) 1,29;
