Analyse descriptive des multiexpositions professionnelles et des maladies respiratoires chroniques dans l’étude CARTaGENE

Contexte : De nombreux travailleurs sont exposés à plusieurs substances et ces multiexpositions peuvent avoir des effets combinés si elles agissent sur le même organe, comme pour diverses maladies respiratoires. Méthodes : Une analyse descriptive des multiexpositions professionnelles a été menée dans la cohorte CARTaGENE comprenant 19 985 participants âgés de 40 à 69 ans au recrutement (2009-2012). Les diagnostics de maladies respiratoires chroniques ont été auto-déclarés par les participants, sauf pour les cas incidents de cancer du poumon, identifiés par le Registre québécois du cancer. Les participants ont rapporté leur profession occupée le plus longtemps et leurs expositions professionnelles ont été déterminées à l’aide de la matrice emplois-expositions CANJEM. Soixante-quatre agents pour lesquels ≥100 participants étaient exposés ont été inclus. Le frequent itemset mining a été utilisé pour extraire les multiexpositions fréquentes. Résultats : Au total, 15 521 participants ont été inclus, desquels 7 370 étaient exposés à au moins un agent et 5 990 étaient multiexposés (médiane = 5 agents). Les multiexpositions fréquentes incluaient : agents de nettoyage – biocides, composés du plomb – HAP et gaz d’échappement – HAP. Des différences dans les profils de multiexposition ont été mises en évidence selon le sexe et les secteurs industriels, mais pas en fonction du statut de maladie. Les hommes étaient plus fréquemment multiexposés que les femmes et les secteurs de la santé, de la fabrication et du commerce comptaient le plus de participants multiexposés. Conclusion : Cette étude constitue un premier pas vers l’élaboration d’hypothèses sur les effets sur la santé de diverses multiexpositions professionnelles.
