Près de 200 000 Canadiens utilisent un fauteuil roulant manuel (FRM), une activité exigeante pour les membres supérieurs et associée à un risque élevé de douleurs et de pathologies de l’épaule. L’entraînement à la propulsion constitue donc un enjeu majeur en réadaptation, mais il demeure peu structuré. Ce mémoire vise à évaluer l’impact d’un entraînement personnalisé, développé en temps réel par un algorithme d’apprentissage par renforcement, sur la biomécanique de la propulsion en FRM. L’étude a été menée sur un simulateur développé au Laboratoire d’Innovation Ouverte en Technologies de la Santé (LIO-ÉTS) et a impliqué 20 personnes en santé et non expérimentées. Six conditions d’entraînement ont été testées, combinant des rétroactions résistives ou assistives-résistives, dans trois objectifs : augmenter l’efficacité mécanique de propulsion (MEF), diminuer le coût physiologique ou combiner ces deux critères. Les résultats montrent que les entraînements résistifs ont permis d’augmenter la MEF moyenne de plus de 6% sur la cohorte, des deux côtés, aux dépens d’un coût physiologique augmentant de +25% à gauche et +17% à droite. Les entraînements assistifs-résistifs ont quant à eux réduit le coût physiologique moyen jusqu’à -34% par rapport à la condition initiale. Ces observations confirment partiellement l’efficacité du modèle de rétroaction personnalisée, capable d’adapter la difficulté de l’effort à chaque individu. En complément, une méthode d’identification inspirée du contrôle a permis d’identifier neuf paramètres intrinsèques et réflexifs caractérisant la dynamique du bras humain. L’intégration future de ces paramètres dans les algorithmes d’apprentissage ouvre la voie à des entraînements adaptatifs et sécuritaires en réadaptation robotisée.
