Abstract
Unplanned dialysis, defined as the initiation of dialysis without a definitive vascular or peritoneal access, remains a significant challenge in the management of end-stage kidney disease (ESKD). Compared to planned dialysis, it is associated with increased mortality, higher healthcare costs, reduced quality of life and productivity.1 A Canadian study reported potential cost savings up to CAD 16.1 million dollars annually with a 50% reduction in unplanned dialysis initiation.2 In Singapore, planned initiation of haemodialysis (HD) stands at a dismal 15%,3 with the authors’ institution, the National University Hospital (NUH), Singapore fairing only slightly better, at 30%. These findings highlight the urgent need for strategies to promote uptake of elective dialysis initiation.
