Поступове старіння населення супроводжується зростанням поширеності когнітивних порушень (КП). За даними численних досліджень пацієнти з цукровим діабетом (ЦД) мають статистично вищий ризик розвитку КП порівняно зі здоровими людьми. Когнітивна дисфункція суттєво ускладнює здатність пацієнтів до самоконтролю, підвищуючи ризик гіпоглікемій та ускладнень. Мета: оцінити ефекти впливу навчання з основ самоконтролю в пацієнтів із ЦД 2-го типу (ЦД2) на тлі ожиріння та КП. Матеріал і методи. Обстежено 140 пацієнтів із ЦД2 віком 25–60 років. За результатами когнітивного скринінгу з використанням короткої шкали оцінки психічного статусу (Mini-Mental State Examination, MMSE) і Монреальського когнітивного тесту (Montreal Cognitive Assessment, MoCA) пацієнти були розподілені на підгрупи залежно від наявності КП та проходження навчання з основ самоконтролю ЦД. Результати. Серед обстежених пацієнтів понад 57% мали легкі або помірні КП. У підгрупах, які пройшли навчання з основ самоконтролю ЦД, спостерігалося вірогідне покращення глікемічного контролю і когнітивних функцій (КФ) (шкала MMSE +1,03±0,16; тест MoCA +0,69±0,12, p<0,001) та зниження рівнів тривоги й депресії за госпітальною шкалою тривоги і депресії (HADS), та опитувальника депресії Бека (p<0,05). Встановлено зворотний кореляційний зв’язок між рівнем глікованого гемоглобіну (HbA1c) та показниками КФ за шкалою MMSE (r=−0,25; p<0,01), а також позитивні асоціації між HbA1c та індексом маси тіла (ІМТ) (r=0,23; p<0,01), показниками тривоги/депресії (відповідно r=0,35; p<0,001 та r=0,30; p<0,001), що свідчить про часте поєднання тривожних і депресивних симптомів у пацієнтів із ЦД2. Висновки. Когнітивна дисфункція є частим ускладненням ЦД2, що призводить до порушення побутової активності, самоконтролю та зниження працездатності пацієнтів. Навчання з основ самоконтролю пацієнтів із ЦД2 сприяє покращенню глікемічного контролю, а також когнітивного статусу та психоемоційного стану. Освітні втручання є ефективним інструментом профілактики прогресування КП у цій категорії пацієнтів.
