Morphological and topographical analysis of delta slow oscillations in propofol general anesthesia using high-density EEG

Les oscillations lentes (OL) dans la bande delta (0,1–4 Hz) constituent une signature électrophysiologique des états d’inconscience, observées aussi bien lors du sommeil profond que sous anesthésie générale. Bien que largement étudiées dans le contexte du sommeil, leurs caractéristiques morphologiques et topographiques demeurent mal définies sous anesthésie générale, en particulier avec le propofol, un agent GABAergique largement utilisé. Cette étude avait pour objectif de (1) caractériser la morphologie des OL induites par le propofol (amplitude, pente, fréquence et fréquence de transition) à l’aide d’un électroencéphalogramme haute densité (hd-EEG, 128 canaux), (2) répartir leur distribution topographique, et (3) examiner leur relation avec l’indice bispectral (BIS), une mesure clinique de la profondeur anesthésique. Des enregistrements hd-EEG ont été réalisés chez six participants adultes durant cinq minutes d’anesthésie stable au propofol avant une chirurgie élective. Un total de 8 193 OL ont été détectées. Les analyses ont révélé des différences régionales significatives : les régions frontales et temporales présentent des amplitudes plus élevées, tandis que la région médiane se distingue par des pentes plus abruptes et des fréquences plus rapides. La corrélation entre les caractéristiques des OL et le BIS est modeste, limitée à une association positive entre la fréquence moyenne des OL et le BIS, sans relation significative lorsqu’analysée uniquement sur les électrodes frontales. Ces résultats démontrent que les OL sous propofol ne constituent pas un phénomène homogène, mais reflètent des dynamiques corticales régionales distinctes. L’hétérogénéité morphologique et topographique observée pourrait offrir des informations complémentaires à celles fournies par les outils de monitorage standard comme le BIS. Cette caractérisation ouvre la voie à l’exploration de nouveaux biomarqueurs électrophysiologiques de la profondeur anesthésique et à des approches innovantes de modulation non pharmacologique de l’inconscience.
