Introdução: Hemorragia subaracnóidea aneurismática (HSA) é um evento de saúde grave, cujas complicações podem apresentar risco para a vida e para a funcionalidade do sujeito acometido. Sua taxa de mortalidade varia entre 30% a 60%, e, aqueles que sobrevivem podem ser acometidos por prejuízos físicos e cognitivos, diminuindo sua qualidade de vida, além de sofrerem com distúrbios de humor, como ansiedade, depressão e estresse pós-traumático. A glibenclamida tem sido estudada como uma opção ao tratamento de lesões cerebrais e parece estar associada a um efeito neuroprotetor ao reduzir o risco de edemas e hemorragias secundárias, colaborando para uma melhor recuperação destes pacientes. Objetivo: avaliar se o uso da glibenclamida em pacientes com HSA poderia trazer impactos positivos para o desfecho do paciente, principalmente em relação ao estado cognitivo, à qualidade de vida e aos aspectos emocionais. Método: pacientes identificados com HSA foram designados aleatoriamente a receber 5 mg de glibenclamida por 21 dias ou placebo, com início do tratamento em um período inferior a 96 horas do ictus. Foram avaliados ainda no hospital em relação à sua funcionalidade (escala modificada de Rankin). Após seis meses, foram avaliados em relação aos aspectos cognitivos, qualidade de vida e estado emocional, com uso da Montreal Cognitive Assessment, Short Form Health Survey, Escala Hospitalar de Ansiedade e Depressão e SPTSS. Resultados: 45 pacientes completaram todas as avaliações. Não foram identificadas diferenças significativas entre os grupos para funcionalidade (p=0,43), estado cognitivo (p=0,329), qualidade de vida (p=0,692), ansiedade (p=0,862), depressão (p=0,652) e sintomas de estresse pós traumático (p=0,641). Conclusão: A glibenclamida parece não ter sido capaz de proporcionar benefícios significativos e visíveis no período de seis meses. A busca por tratamentos medicamentosos que minimizem os danos da HSA ainda é necessária
