Introdução: Achados recentes sobre os benefícios da glibenclamida, como a significante redução em danos neurológicos observados em modelos de isquemia cerebral, traumatismo craniano e hemorragia subaracnoide (HSA), com menor formação de edema e menor apoptose de neurônios hipocampais, assim como amenização significativa de danos à memória espacial, iniciaram uma nova era para estudos prospectivos em sulfoniluréias. Objetivos: Avaliar o papel da glibenclamida na evolução clínica de pacientes com HSA aneurismática, assim como estudar o impacto da glibenclamida na taxa de mortalidade, qualidade de vida e performance cognitiva em pacientes com HSA aneurismática. Métodos: Pacientes com confirmação radiológica de HSA proveniente de aneurisma cerebral, idade entre 18 e 70 anos e tempo de ictus inferior a 96 horas, foram alocados aleatoriamente em dois grupos, um para receber 5 mg de glibenclamida por 21 dias e outro para controle com placebo. Informações clínicas gerais, escala modificada de Rankin (mRS) na alta e após 6 meses de seguimento e o status cognitivo tardio (através do questionário SF-36 (Short Form Health Survey); Montreal Cognitive Assessment (MoCA) foram obtidos em ambos grupos. Resultados: 78 pacientes foram randomizados. Ambos grupos de alocação tiveram uma caracterização clínica semelhante. O grupo de intervenção apresentou menos ocorrência de vasoespasmo clínico (p=0,038) em análise univariada, mas esta diferença não se manteve na análise multivarida (p=0,202). Não houve diferença entre os grupos em seu valor de mRS na alta, como também após 6 meses. Não houve diferenças estatísticas entre os grupos na análise neuropsicológica com os testes SF-36 e MoCA (p= 0,887). Conclusões: Não foram observadas diferenças de desfecho cognitivo e funcional, assim como mortalidade em relação ao grupo placebo. Novos ensaios, maiores e multicêntricos, são necessários para se saber o exato papel desta medicação no tratamento da HSA
