Prevalência de xerostomia em portadores de esclerose sistêmica : revisão sistemática e metanálise

Abstract
A esclerose sistêmica é uma doença rara do tecido conjuntivo, de etiologia desconhecida, caracterizada pelo espessamento progressivo da pele e dos órgãos internos e vasculopatia. O acometimento orofacial é comum, sendo a xerostomia um sintoma frequente nesta população, possivelmente secundária à fibrose glandular, mas sua frequência permanece desconhecida. O objetivo deste trabalho é analisar a prevalência de xerostomia em indivíduos com diagnóstico de esclerose sistêmica. Após uma busca extensiva nas bases de dados (PubMed, Embase, Scopus, Web of Science e CINAHL e literatura cinzenta), os títulos e resumos foram avaliados por dois autores independentes. Os artigos selecionados tiveram os textos completos analisados e os que atenderam os critérios de elegibilidade foram incluídos. Dois revisores avaliaram independentemente o risco de viés nos estudos incluídos, usando a escala de Newcastle Ottawa. Foi realizada uma meta-análise de efeitos aleatórios usada para reunir estimativas de prevalência de xerostomia dos pacientes com esclerose sistêmica. A heterogeneidade foi avaliada através do I2. Foram analisados possíveis viés de publicação usando o teste de Egger, e foi gerado um gráfico de funil. Foram incluídos 10 estudos com 522 pacientes com ES, entre estes, 253 apresentaram xerostomia (50,51%, IC95%=27.12 - 73.79). A quantidade de indivíduos com xerostomia nos estudos analisados variaram de 1.82% a 100% da amostra, demonstrando grande heterogeneidade e alto risco de viés. Foi possível observar que metade dos dos indivíduos com esclerose sistêmica relataram queixa de xerostomia. O estudo encontrou uma substituição considerável do tecido glandular por tecido fibroso, o que contribui para a redução do fluxo salivar.
